Roper St. Francis Healthcare has positions for Nurse Practitioners and Physician Assistants in the following areas:
Post Acute Care - clinic and residential facility
Full Time and Part Time Advanced Care Practitioners will provide on-going medical management for acute/episodic, rehabilitation patients, as well as long term chronically ill patients. This is done in an onsite Geriatric Primary Care Clinic for independent residents and within a Residential Healthcare facility for short term and long-term residents. Supervising physician is also on site daily.
The APP will work as a member of the interdisciplinary team of the facility to effectively designate processes for meeting the medical, psychosocial and spiritual care needs of the patient, resident. The APP will develop updates and evaluate medical treatment plans, medications, and other necessary care.
Other Sites are in Mt.Pleasant and Downtown Charleston. APP will also be part of a larger Healthcare System with access to specialists and other services as part of that network.
EMR - EPIC and Point Click Care
Contracts for mobile radiology, lab, Wound Care Specialist, Hospice Care and Infusion Specialists in place to support the facility providers.
Weekend call availability is required. Call rotation is 1:3 at present but will expand
