Job Overview
We are seeking a dedicated and compassionate Memory Care Coordinator to oversee our memory care program. This role is essential in ensuring that our residents receive the highest quality of care in various settings. The Memory Care Coordinator will be responsible for overseeing care plans, coordinating services, and providing leadership to ensure that all aspects of care are effectively managed.
Duties
- Developing and implementing care plans for patients with memory-related conditions.
- Manage any behavioral or health-related incidents
- Coordinating staff and monitoring patient progress.
- Ensuring a high standard of care.
- Planning and implementing individual and group activities based on residents' needs and interests.
- Staffing the activities department and hiring/training new employees.
- Coordinate with healthcare professionals, families, and caregivers to ensure seamless communication and collaboration in the delivery of care.
- Provide guidance and support to staff in areas such as dementia care
- Conduct regular assessments of client needs and adjust care plans as necessary.
- Advocate for clients' rights and preferences while ensuring adherence to safety protocols.
Experience
- He/She must have a deep understanding of memory impairments and the best practices in caring for individuals with such conditions.
- Previous experience in senior care or related fields is preferred, with a strong understanding of Alzheimer's
- At least a CNA certified
- CPR certified
- Proven leadership skills with the ability to motivate and manage a team effectively.
- Familiarity with dementia care practices is a plus.
