Care Management Associate Job Overview :
The Care Management Associate utilizes critical thinking and judgment to collaborate and inform the case management / concurrent review process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination, support and education for members through the use of care management / concurrent review tools and resources. They will assist with discharge plans, and outreach to members, assist with HH / DME issues.
Position Summary :
Authorizations will need to be entered within 24 hours of notification (BH IP, SUD Resi, Detox, Subacutes, BHRF, BHIF, HCTC, COE / COT, BH SNF, Retrospective and UPC)
Certification of Needs (CON) and Recertification of Needs (RON) needs to be researched and filed. DD Notifications needs to be sent to DD Coordinators upon admissions
Assigned and label faxed medical records to correct clinician by assigned facilities
Assist with Discharges to be entered and research ASAM scores
Industry : Healthcare
