Position Summary
Oversees care coordination for Rosecrance clients throughout the state of Illinois to ensure clients and families are linked to services and supports to decrease hospitalization, crisis episodes and to increase community functioning for the client to achieve rehabilitative, resiliency and recovery goals. Provide continuity of program services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Serve as the lead contact and coordinate clinical efforts related to CCBHC programming, Illinois Health Practice Alliance (IHPA), ADT, and improve the integration of behavioral and physical health care in the state and other statewide value-based care. In addition to treatment services, responsibilities of the position include assertive case management, maintaining an overall perspective on client functioning, environmental factors, resource needs and advocacy on behalf of clients. The aim of services is to work with the clients (and families/others) toward supporting their ability to assume responsibility and manage their emotions and behavior and to thereby function in the least restrictive setting possible.
This role provides but is not limited to case management services, crisis intervention, skills training, documentation within the required time frame, individual and group psychoeducation sessions, medication monitoring, attends meetings, performance improvement and other duties that are necessary to meet the needs of our clients.
Requirements
- Bachelor’s Degree in Psychology, Social Work, Counseling and Guidance, Rehabilitation Counseling, Vocational Counseling, Pastoral Counseling, Family Therapy or related field. Master’s degree in a behavioral health related field preferred
- Must qualify as a Mental Health Professional (MHP) or higher in accordance with IL DMH Clinical Designation Rule 132
- Demonstrated experience, education, or training in behavioral health services
- Working knowledge of substance use disorders and symptoms as identified in the DSM 5
- Adequate written skills to accurately complete required documentation within the time frames prescribed
- Competence in using common computer applications such as word processing, spreadsheet, and presentation software
- Valid driver’s license and the ability to meet agency insurance requirements

Responsibilities
- Adhere to prescribed work hours set by leadership, and any additional hours deemed necessary for the responsibilities assigned.
- Perform supervisory duties as assigned.
- Use available data to monitor effective team behavior in compliance with standards.
- Develop and implement a care coordination system that adheres to the fidelity of the Certified Community Behavioral Health Clinic (CCBHC) model of care.
- Gain a comprehensive understanding of value-based contracting and incentive structures, as well as the Illinois Health and Payor Alliance (IPHA) platforms, to effectively manager and optimize contractual agreements and performance metrics.
- Attend all scheduled meetings of staff, such as consultation, team meetings or appropriate committee meetings as designated.
- Engage in staffing sessions to evaluate client progress, discuss treatment outcomes, and determine future clinical directions and strategies.
- Conduct assessments and facilitate educational sessions for clients and their family members to support their understanding and engagement in the care process.
- Formulate initial diagnostic impressions and provide appropriate recommendations based on assessments and evaluations.
- Motivate and collaborate with staff to address and resolve issues related to effective clinical practices and improve overall team performance.
- Demonstrate self-motivation and a proactive approach to enhancing performance, problem-solving, and address the complex needs of clients and their families.
- Adapt verbal and written communication to suit the language and style of clients and their families, ensuring clarity and effectiveness in interactions.
- Complete and submit all required documentation within specified timelines to ensure compliance and accuracy.
- Undertake primary case management responsibilities for designated clients.
- Maintain communications with all staff required for care coordination.
- Maintain knowledge of community, school and recovery resources in order to make appropriate interagency referrals as required
- Utilize a variety of treatment concepts, evidence-based practices and service modalities when providing care.
- Present workshops, lectures and training to clients, families, staff, and community groups as needed.
- Exercise confidentiality in keeping with the professional Code of Conduct and within the framework of the law.
- Deliver exceptional customer service consistently to every customer.
- Serve as a role model and demonstrate positive guest relations in representing the agency.
- Assume other related responsibilities as assigned by leadership.

- Hours: 8:30am - 5:00pm (40 hours per week)
- Days: Monday - Friday

Compensation & Rewards
- Base Pay: Starting at $43,908.80/year (pay is based on education, experience, and credentials)
