Position Summary:
We are seeking a Program Care Manager - MSW or LADC to play an exciting and vital role in expanding the EMPOWERED Program from Clark County to Northern Nevada. The EMPOWERED program currently provides support to pregnant and postpartum individuals experiencing a substance use disorder with a tailored, person-centered approach designed to empower its participants to be prepared for the birth of their babies and to thrive as a caregiver. The program links participants to prenatal and substance use treatment as needed, and to community resources that address the social determinants of health. The expansion of this program will focus on Washoe County, Carson City, Churchill County, Lyon County and Storey County.*This position is on-site in order to ensure optimal collaboration, efficiency, and teamwork.
Essential Functions:
- Establishes and maintains partnerships with community organizations to recruit pregnant women with substance use disorder and provide service to the community.
- Develops and implements activities that keep women enrolled in the program engaged with services throughout their pregnancy and during the one-year postpartum period.
- Responsible for intake, performing a comprehensive assessment and developing a treatment plan to facilitate care coordination and linkage to care.
- Completes required documentation and paperwork in a timely manner.
- Ensures compliance with policies and standard operating procedures.
- Participates in educating future physicians and other health care professionals by providing expertise and guidance in developing and launching community-based student projects.
- Assists with obtaining and compiling feedback from community stakeholders, key university personnel, and stakeholders, including the preparation of annual report, and outreach outcomes to facilitate program evaluation.
- Represent Roseman University and EMPOWERED at various community events, such as health fairs, and development activities.
Qualifications:
- Master’s degree in Social work, two (2) years related experience, or any combination of education, experience and training which provides the following Knowledge, Skills, and Abilities.
- Experience involving community outreach working with pregnant and postpartum women preferred.
- Knowledge and experience working with co-occurring populations (chronic health conditions, mental health, and substance use) a plus. Knowledge of diverse populations that speak English/Spanish or English/Asian Pacific Island language preferred.
- Criminal background, driver’s license, and fingerprinting required at employment.
