JOB TITLE: Licensed Practical Nurse (LPN)
REPORTS TO: Director of Nursing/Nurse Manager
FLSA STATUS: Non-Exempt
JOB OVERVIEW: Provides direct and indirect care in the inpatient rehabilitation setting. Communicates with physicians, DON, Therapy Dept., and co-workers, as appropriate about changes in patient?s clinical condition including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment. Additionally, is able to perform general nursing duties and functions while under the supervision of the RN and within the scope of the LPN practice acts.
ESSENTIAL FUNCTIONS INCLUDE BUT ARE NOT LIMITED TO:
a. Knowledgeable of medications and their correct administration based on age of the patient and their clinical condition.
b. Follows the five medication rights and reduces the potential for medication errors.
c. Performs patient care responsibilities considering needs specific to the standard of care for patient?s age.
d. Demonstrates ability, under direct management of RN, to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes adult, geriatric and the general patient population.
e. Able to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
f. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
g. Able to revise plan of care as indicated by the patient?s response to treatment and evaluate overall plan daily for effectiveness.
h. Integrate the nursing plan of care with the Therapy Department treatment plan to attain the patient?s optimal level of functioning.
i. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate.
j. Demonstrates ability to perform treatments and provide services to level of licensure.
k. Communicates appropriately and clearly to team leader, co-worker, physicians and the Therapy Department.
l. Consults with therapy staff daily to provide for an interdisciplinary approach to the patient?s needs and documents patient?s status to include rehabilitation therapy progress.
m. Provides care appropriate to condition and age of the patient, including adult, geriatric and general population.
n. Demonstrates an ability to assist physicians with procedures and performs services requiring technical and manual skills under the direction of an RN.
o. Manages and operates equipment safely and correctly.
p. Obtain intravenous access as needed and administer intravenous medications according to scope of practice.
q. Perform veinapuncture according to order to assist physicians with care and diagnosis.
r. Perform skin/wound assessment every shift. Provides wound care as ordered by physician or other licensed provider.

z. Maintains stock supply levels on the unit. Notifies charge nurse or DON when supplies are low.
aa. Orders medication from the pharmacy ensuring that OTC medications are not ordered and an adequate supply of PRN medication are available.
bb. Documentation meets current standards and policies.
cc. Identifies and addresses psychosocial needs of patients and family; communicates with Social Services/Discharge Planner regarding both in hospital and post discharge needs.
dd. Coordinates and supervises patient care under the direct supervision of a RN.
ee. Medical and Surgical asepsis is carried out during treatments and special procedures.
ff. Respond to the patient as appropriate in emergency or physically distressful situations.
gg. Skills Checklists/Competencies submitted timely.
