Provides direct and indirect care in the inpatient rehabilitation setting. Communicates with physicians, DON, Therapy Dept., and co-workers, as appropriate about changes in patient's clinical condition including results of diagnostic studies and symptomatology. Can respond quickly and accurately to changes in condition or response to treatment. Position Overview:

Key Responsibilities:
- Knowledge of medications and their correct administration based on age of the patient and their clinical condition.
- Follows the five medication rights and reduces the potential for medication errors.
- Performs patient care responsibilities considering needs specific to the standard of care for patient's age.
- Demonstrates ability, under direct management of RN, to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes adult, geriatric, and the general patient population.
- Able to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Able to revise plan of care as indicated by the patient's response to treatment and evaluate overall plan daily for effectiveness.
- Integrate the nursing plan of care with the Therapy Department treatment plan to attain the patient's optimal level of functioning.
- Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate.
- Demonstrates ability to perform treatments and provide services to the level of licensure.
- Communicates appropriately and clearly to team leader, co-worker, physicians, and the Therapy Department.
- Consults with therapy staff daily to provide for an interdisciplinary approach to the patient's needs and documents patient's status to include rehabilitation therapy progress.
- Provides care appropriate to the condition and age of the patient, including adult, geriatric and general population.
- Demonstrates an ability to assist physicians with procedures and perform services requiring technical and manual skills under the direction of an RN.
- Manages and operates equipment safely and correctly.
- Obtain intravenous access as needed and administer intravenous medications according to scope of practice.
- Perform venipuncture according to order to assist physicians with care and diagnosis.
- Perform skin/wound assessment every shift. Provides wound care as ordered by physician or another licensed provider.
- UMRH Performance Evaluation
- Demonstrates competency in the following areas: Duties and Responsibilities.
- Treats patients and their families with respect and dignity; ensures patient confidentiality.
- Demonstrates an ability to be effective, flexible, well organized and function well under stressful situations, to provide proper patient care.
- Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
- Maintains a good working relationship both within the department and with other departments.
- Maintains stock supply levels on the unit. Notifies charge nurse or DON when supplies are low.
- Orders medication from the pharmacy ensuring that OTC medications are not ordered, and an adequate supply of PRN medication is available.
- Documentation meets current standards and policies.
- Identifies and addresses psychosocial needs of patients and family; communicates with Social Services/Discharge Planner regarding both in hospital and post discharge needs.
- Coordinates and supervises patient care under the direct supervision of an RN.
- Medical and Surgical asepsis is carried out during treatments and special procedures.
- Respond to the patient as appropriate in emergency or physically distressful situations.
- Skills Checklists/Competencies submitted timely.
- Follows United Medical Hospitals' Corporate Compliance Code of Conduct as evidenced through practice, performance, and attendance of annual review programs/in-services. ii. Performs other duties as assigned within the LPN scope of practice.
- Current Louisiana LPN Licensure
- BCLS Certification
- Prior experience in Rehabilitation is highly desirable but not mandatory.
