JOB TITLE: Nurse Liaison
REPORTS TO: Administrator
FLSA STATUS: Exempt
JOB OVERVIEW: Responsible for the efficient coordination of the admission process from initial referral contact to patient presentation for admission. Activities include communication with referral source, administration, nursing department, and medical director throughout the evaluation process, and transportation arrangements for admission. Also responsible for the development and implementation of business and marketing plans for the facility. Participates in the marketing plan process through market research, market analysis and marketing tactics. Works on new existing facility programs. Participates in the department’s performance improvement activities.
ESSENTIAL FUNCTIONS INCLUDE BUT ARE NOT LIMITED TO:
1. Maintains accurate list of referral sources, with addresses and telephone numbers.
2. Receives calls from referral sources and responds to all referrals immediately, but no later than 60 minutes.
3. Provides HIM with all necessary information on referrals and admissions.
4. Completes accurate admission assessment and presents to admissions team, either in person or by telephone, within 1 hour of referral. Provides copy of pre-admission screen to Rehab Program Coordinator and obtains physician acknowledgement and approval (or denial) of pre-admission screen prior to patient admission. Revises pre-admission screen information if patient is not admitted within 48 hours of initial evaluation.
5. Obtains pre-authorization for patients with commercial insurance.
6. Calculates cost of medications for potential patients and reports to admissions team.
7. Responds with a decision to referral source immediately, but no later than 30 minutes, of a decision by physician and admissions team.
8. Alerts nursing and therapy departments of anticipated admissions within 30 minutes of decision to admit, giving report to charge nurse with anticipated admission time.
9. Arranges for transportation for admission.
10. Obtains clinical updates, reports of patient progress and discharge summaries and send to referral sources (if requested) within 24 hours.
11. Conduct in-services and report to administration the date, place and audience of the in-service.
12. Provide Administration with a written weekly report of referrals, preAdmission screens and admissions, denials with reasons and with copies of thank you notes to referral sources.
