Responsibilities
- Oversees all day-to-day functions of our Memory Care Program to include resident care and team members.
- Responsible for development of individual service plan for new residents and ongoing review/updating of individual service plans in response to changes in resident needs.
- Responsible for completion of Uniform Assessment Instrument (UAIs).
- Coordinate management of all healthcare needs of residents with physicians, other healthcare providers and families.
- Responsible for ensuring that assigned team members are fully aware of resident needs and individual service plans.
- Ensures resident charts are current and organized and required documentation is completed. Performs audits as needed.
- Supervises medication aides to ensure proper administration and monitor documentation.
- Supervises direct care team members to ensure resident needs are met in a timely, courteous, professional, and caring manner.
- Assist with scheduling as needed to ensure proper staffing levels.
- Participate as a Manager on Duty during assigned weekends. Be on call for resident oriented situations.
- Foster teamwork and have a positive impact on team members, residents, and visitors.
Qualifications
- Must be 21 years of age or older.
- Must read, write, speak and understand English.
- Bachelors Degree in Social Work, Psychology, Gerontology, Therapeutic Recreation, or equivalent is desired.
- Current RN/LPN license strongly preferred.
- Previous successful management of a Memory Care unit is desired.
- Knowledge and experience in working with older adults with memory impairment; at least two years experience in geriatric care is desired.
- Ability to communicate effectively.
- Must be able to pass a criminal background check.
