Summary: The Wellness Coordinator Assistant will provide quality, respectful, courteous service to all individuals served. An overview of responsibilities includes but not limited to, completing Government Performance and Results Assessments (GPRA), assisting clients with the wellness program activities, and completing administrative tasks. The Wellness Coordinator Assistant will create a healthy environment for program participants and focus on helping them to improve all areas of their lives while in recovery.
Supervisory Responsibilities: No
Duties and Responsibilities:
1. Observe all rules of confidentiality relating to clinical information and treatment within the agency and when communicating with external agencies or individuals.
2. Assist Wellness Coordinators in conducting Government Performance and Results Assessments (GPRA) at designated times and complete documentation to be entered into a statewide reporting system.
3. Assist Wellness Coordinators in obtaining an 80% follow-up rate on all completed GPRA surveys.
4. Actively identify and support linkages to community resources that support the clients’ goals and interests. This will involve a collaborative effort including the client, agency staff, and other relevant community partners.
5. Assist clients in increasing social support networks of relatives, friends, and/or significant others.
6. Link clients to appropriate professional resources when needed.
7. Link clients to treatment and peer support services as needed and refer to RCCAC’s CARES Program.
8. Maintain communication with the Recovery Support Services staff relating to participant engagement during wellness activities.
9. Ensure clients have access to all forms of Medication Assisted Treatment (MAT).
10. Maintain contact with clients upon discharge from the program to ensure their ongoing success and to provide re-engagement support if needed.
11. Use computer skills and software to enter and manage program data. Assist Wellness Coordinators with tracking attendance, participation, and performance data related to the wellness program.
12. Submit success stories and activity reports monthly.
Skills/Abilities:
1. Must have a demonstrated commitment to the recovery community.
2. Communicates well both verbally and in writing, creates accurate and punctual reports, delivers presentations, shares information and ideas with others, and has good listening skills.
Education/Experience:
1. High School Diploma or equivalent (required).
2. Experience with substance use recovery, case management, and office administration preferred.
3. Proficiency in Microsoft office suite.
Physical/Employment Requirements:
1. Ability to lift up to 40 pounds.
2. May work in a variety of environments including professional offices, treatment providers, outreach and residential centers, and community events.
3. Pass pre-employment screenings.
4. Must have reliable transportation, and insurable under agency insurance policy.
