SUPERVISE: Habilitation Specialist 1/Habilitation Assistant/Direct Support Professionals

FUNCTIONS

The occupant of this position will be primarily responsible for the day to day operation of the day programs and vocational facilities to include but not limited to client assessment, program development, program implementation, program documentation, program modifications, program integration, staff development, procurement and maintenance of day program supplies/equipment.

This also includes the implementation of each clients’ Individual Habilitation Plan through compliance in the delivery and documentation of services and activities for the clients’ habilitation/vocational needs. The Habilitation Specialist will assure facilitation of a harmonious relationship within the day program between staff and clients. It is the Habilitation Specialist’s responsibility to approve changes to the day program schedule.

Other duties may be assigned as necessary.

ESSENTIAL JOB DESCRIPTIONS

  • The major responsibilities of the Habilitation Specialist will include the following and the other task deemed appropriate by the Executive Director.
  • Systematically monitors the implementation of the individual support plan and the overall progress in order to maximize the physical intellectual and vocational potential of the client.
  • Maintain current knowledge of ICF-IDD regulations to ensure program compliance provide daily management of all day program activities.
  • Duties/Work Plans:
  • The duties and/or work plans of the Habilitation Specialist include but are not limited to the following:
  • Develop day program curriculum in accordance with client needs and in conjunction with the interdisciplinary team.
  • Serve as a member of the interdisciplinary team.
  • Attend other programmatic meeting, conferences, workshop, etc. at the direction of the Administrator.
  • Develop day program objectives and integrative activities based on the needs of the client in accordance with the recommendations of the interdisciplinary team.
  • Ensure all programs conducted in the day program are implemented in accordance with the recommendations of the interdisciplinary team.
  • Enter monthly, quarterly, and event log notes to maintain compliance with ICF/MR guidelines.
  • Ensure continuous active treatment is conducted.
  • Maintain update Master Day Program Schedule
  • Approve day program special events.
  • Monitor active treatment at all off site Day Programs Inc., Support Employment Assessments and/or Employer Follow-Ups
  • Coordinate Programs and/or Activities to Provide Continuum Services with Client’s ISP
  • Orientation\Training of new Day Program Staff
  • Preliminary 45, 90, and Annual Performance Evaluations for Habilitation Specialist I, Habilitation Assistants and Instructors
  • Validate at least five Formal Training Objectives per bi-weekly
  • Provide Administrator with weekly reports of Habilitation Specializing Activities, including but not limited to the following:
  • Copies of Assessments Completed
  • Programs In-Serviced
  • Program Validations (Minimum of 5 per bi-weekly)
  • Supportive Employment Follow-up
  • Individual Support Plans Attended
  • Copies of Education/Vocational Evaluations
  • Client progress notes, programs written, and/or modified
  • Core Team Needs/Recommendations/Follow-up Needs
  • Supplies and/or Materials Needed
  • Inspection Reports for Day Programs (Sanitation, Safety Concerns, Infection Control Issues, etc….)
  • Attend and conduct, as assigned, regularly scheduled facility and agency in-services
  • Assist in orientation, training and mentoring of other Habilitation staff as requested
  • Participate in direct support staff training on each client program objectives prior its initial implementation
  • Ensure habilitation staff maintain certifications for medication administration, NCI, first aid and CPR
  • Maintain training to meet clients’ individual needs including but not limited to medical procedures, therapies and/or behavioral support plans
  • Ensures that each person served has an activity schedule developed and implemented.
  • Monitors and ensures staff follows procedures to promote optimum health care and behavior supports, including the implementation of services outlined in person(s) served service plan, and staff training.
  • Monitors and ensures staff completes all documentation as required to ensure quality services.
  • Maintains a work environment that supports dignity and fair treatment of staff.
  • Participates in recruitment, ensures vacancies are filled in a timely manner, and participates in hiring and termination decisions for assigned service sites.
  • Conducts timely performance reviews.
  • Reviews and provides feedback on all staff documentation
  • Serves as a positive role model and supports development of staff.
  • Responds to emergencies for person(s) served.
  • Participates as needed with investigations involving persons served and employees at assigned service sites.
  • Monitors and ensures staff follows procedures to promote optimum safety practices to provide a safe environment
  • Must possess a valid NC Driver’s License;
  • First Aid and CPR Training;
  • Must be certified to administer medication within 90 days of employment.

SPECIFIC

  • Ensure adherence to all CARF, State and MCO standards for Mental Health licensed agencies
  • Maintain Confidentiality and HIPPA laws for all RGH clients in accordance to facility policies and state regulations
  • Promote awareness and carry out the organization’s mission and vision throughout services rendered
  • Assure organization’s strategic plan is implemented within the day to day operation of the group home
  • Promote a homogeneous environment between RGH staff, clients and community supports
  • Maintain professional and ethical decorum at all times
  • Ensure day program’s operational budget is within the budgetary guidelines established by the board approved annual budget
  • Provide direction and leadership to staff to ensure continuity of services throughout the day program
  • Ensure that dignity/respect and quality of life is maintained at the highest level for all clients
  • Adhere to all facility policy and procedures in accordance the organizations’ guidelines
  • Provides on-call support in the absence of habilitation staff
  • Ensure that day program and vocational settings maintain adequate inventory and staples to meet the needs of the clients served
  • Maintain certifications and required professional development through training seminars and conferences
  • Participate on meeting with interdisciplinary team in providing for overall care for each client
  • Ensure teachable moments are incorporated throughout the each client treatment plan in the day program and vocational settings
  • Notify the Clinical Director and Executive Director in the event or suspension of client abuse, neglect or exploitation is expected
  • Ensure Incidents, Medication Errors, Shift/Goal Notes and/or other documentation is completed and recorded daily
  • Immediate report to Clinical Director if Incident and/or Medication Error occur in the day program or vocational settings.
  • Provide comments or suggestions for the optimum service implementation and/or organization improvement
  • Receive, review, process, file and submit a variety of documents and reports accurately and in a timely manner

CLIENT SERVICES

  • Ensure implementation of client’s ISP in accordance to ICF/IDD regulatory guidelines
  • Carry out each clients’ treatment plan in a developmentally appropriate manner to meet the needs of each client served
  • Serve as an advocate for each RGH client in the group home and community to ensure they exercise their rights to the fullest extent and provide assistance, as needed for rights they cannot fully execute independently
  • Administer client medication as ordered by the physician and/or specialist
  • Ensure clients are free from abuse, neglect and/or exploitation
  • Ensure implementation of client’s day program activity schedules remain current and reflect the interests/desires of the client
  • Ensure that clients are supervised at all times unless otherwise specified in their Individual Support Plan
  • Provide ongoing collaboration with Clinical Director, QIDPs and other Interdisciplinary Team Members to maintain good overall health for clients with notification of changes and/or improvements to treatments prescribed
  • Ensure ongoing collaborations with day program staff and community representatives are delivered in a professional and positive manner
  • Assess needs; develop training objectives to address educational and vocational skills annually
  • Ensure doctor’s orders are followed as written and adaptive equipment is always in good repair
  • Conduct and document meetings to provide ongoing client training, notification of activities, and/or related opportunities available for them at RGH and in the community
  • Ensure complete, accurate and quality documentation for each assigned client
  • Facilitate, transport and collaborate with health care professionals to provide continuity in clients’ wellness
  • Provide input in the evaluation of each assigned client for his/her Adaptive Behavior Inventory
  • Ensure documentation and notification to Clinical Director and Quality Assurance Coordinator of changes in clients’ independent living strengths and needs
  • Ensure Habilitation Assistant are implementing habilitation training objectives, health care procedures and service goals as needed

PROGRAM OVERSIGHT DUTIES

  • Ensure implementation and oversight of facility safety standards and regulations in the day program
  • Conduct and document House Fire and Tornado Drills monthly
  • Adhere, enforce and participate in the implementation and ongoing oversight of all RGH safety standards and regulations
  • Performs emergency medical procedures as needed in accordance to RGH Emergency Management Procedures and OSHA regulations
  • Ensure a clean, safe and orderly environment by performing general cleaning and housekeeping
  • Ensure maintenance and/or outside contractors keep appliances and equipment functional and operational

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Maintain client finances, liability certification and client review weekly.

  • Physical Requirements

Must be able to operate a variety of equipment (e.g. wheel chairs, lifts, etc.). Must be able to exert up to seventy-five pounds of force occasionally and/or frequently to lift, carry, push, pull or otherwise move objects. Physical demands are in excess of sedentary work; position requires walking, standing, reaching, fingering, stooping, etc. Will lift no more than forty pounds independently. Must be able to lift and/or carry weights of up to one hundred and seventy-five pounds with assistance.

QUALIFICATIONS

  • Bachelor’s Degree in Social Work or Education from an accredited program or a Bachelor’s Degree in a related field and at least three (3) years of experience developing programs for developmentally delayed individuals.
  • Two years Group Home experience working with individuals with intellectual and development disabilities in a residential setting is preferred.
  • Previous experience providing services and supports to individuals with developmental disabilities and/or related disorders preferred.
  • Ability to read and write detailed notes and instructions
  • Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company.
  • Possess a valid NC Driver’s License
  • Computer Skills - Must have basic work processing (MS Word) and email usage skills.
  • Operate a variety of office and other equipment (e.g. copier, telephone, vacuum, food preparation equipment, etc).
  • Reasoning Ability - Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Experience in managing systems, processes, and people.
  • Must be able to work independently as well as part of a team.
  • Ability to work flexible hours and locations, including covering direct care shifts as necessary.
  • Capable of working responsibly with highly confidential information.
  • Available to work a flexible work schedule including weekends

WORK ENVIRONMENT AND PHYSICAL DEMANDS

  • Physical demands: Must be able to utilize proper body mechanics while lifting up to 75 pounds, must be able to bend, stoop, push, pull reach, sit, and walk for periods of time. Must have fine vision, sustained vision, and peripheral vision. Must possess sufficient eye/hand coordination to operate office, adaptive, and household equipment.
  • Work environment: Light and ventilation as found in typical home or office setting. Hazards may exist if the employee fails to use proper body mechanics when lifting or transporting an individual or item, if vehicles are not operated safely, or if proper techniques are not used when intervening with individuals. The possibility of blood borne pathogens exists if universal precautions are not followed.
