We are a medical adult day care center serving seniors in Middlesex County and nearby areas. We provide our participants with nursing oversight, social work support, engaging activities, and other services during a 5-hour day program, which is designed to help seniors at risk for decline maintain their independence as long as possible.
Role Description
Immediate opening : We are seeking a New Jersey Certified Social Worker, part-time to start with potential for a full-time work as facility census grows. The Social Worker will be responsible for providing Social Work assessments, care planning, counseling and connection to community services to participants of the program, plus other varied duties. Training provided.
Qualifications
- New Jersey Certified Social Worker (at a minimum)
- Experience with seniors and elder care services helpful, particularly in health care settings
- Knowledge of standard social work principles and practices
- Ability to perform participant assessments and care planning in conjunction with interdisciplinary team
