Company Purpose/Mission:

Roy Maas Youth Alternatives (RMYA) creates brighter futures for children in crisis by promoting individual success and healthy relationships in a safe, healing environment, giving children and families the tools to end the cycle of abuse. RMYA provides treatment programs for traumatized children who most often are victims of sexual, physical, and/or emotional abuse. RMYA supports using trauma-informed techniques and relationship-based approaches to create a safe environment that provides consistency, structure, and caring for the children we serve.

Job Summary:

Case Manager II (CM-II) provides specialized and intensive case management services to homeless and at risk youth (18-24) by collaborating with those individuals’ seeking services through outreach and transitional living programs. CM II will ensure immediate needs (food, shower, shelter, etc.) are met and will develop and implement a plan that helps clients receive the resources needed to successfully obtain education, employment, mental health and stable living environment goals  in a safe placement. The position requires coordination with various government agencies, city community groups, civic and non-profit organizations and RMYA resources to connect youth to needed services.

Essential Duties/Responsibilities:

Case Management:

  1. Engage with and develop rapport with homeless, DFPS extended care, and community youth to serve them.
  2. Complete all intake and assist youth with the apartment application.
    1. Assure immediate needs are met and the apartment is ready for their move-in.
    2. Connect youth with Clinician and other resources.
    3. Complete an initial needs assessment and act upon critical needs appropriately and immediately Perform Commercial Sexual Exploitation Identification Tool (CSE-IT) assessments if needed. This is done by clinician
    4. Refer clear concern qualified youth to care coordination. Counselor does this.
    5. Connect youth with Clinician and other program resources.
  3. Develop and implement client driven LIFE plans that promote moving clients toward self-sufficiency:
    1. Identify both short-term and long-term goals.
    2. Develop timelines and measures for each goal.
    3. Empower clients to become involved in their own planning and goal setting.
    4. Resource Management: becoming familiar with the resources in the area and referring based on the needs of the client.
    5. Monitor progress towards goals in regularly scheduled weekly CM sessions.
    6. Maintain weekly contact with youth to encourage program/service participation.
  4. Ensure recordkeeping and reporting guidelines are followed:
    1. Enter and maintain data into relevant Apricot management systems.
    2. Maintain client files for services provided.
    3. Report critical incidents immediately to the Sr. Director of Shelter & Transitional Living
    4. Collect data necessary to meet funding requirements and statistical reports.
    5. Complete all grant and agency required reports in a timely manner.
    6. Ensure client documentation is complete, accurate, and timely.
  5. Coordinate Life Skills and other training programs aimed at learning and practicing life skills and decision-making.
  6. Coordinate and implement day and evening programs, schedule outside service providers and volunteers.
  7. Attend weekly staffing meetings regarding client case management.

Program Management:

  1. Ensure the safety and health of all clients by implementing, monitoring, and applying guidelines for apartment accommodations, and cleanliness and maintenance requirements of the apartment management by:
    1. Conducting tours of the apartment facilities as needed
    2. Inspecting for any hazards or repair needs
    3. Reporting any hazards to the Sr. Director immediately/ apartment management.
    4. Communicating with the resident managers to ensure safety for the residents.
    5. Facilitating the repair process in collaboration with the apartment property manager.
    6. Follow up with apartment complex on maintenance tickets submitted.
    7. Ensuring facilities are maintained in accordance with established client contracts (e.g., basic upkeep to include cleanliness, timely disposal of trash, completion of everyday chores, etc.)

Outreach:

            Work in harmony with the Outreach Manager to -

  1. Develop community and other agency contact information, notes, and reports determined to be necessary as part of the comprehensive efforts of the program.
  2. Compile and utilize data to support communication with prospective community partners and aid in the development and implementation of outreach policies and procedures.
  3. Attend community events focused on youth exploitation to promote RMYA Programs and Resource
  4. Develop and maintain community and target client information resources and intelligence to determine where and when to do outreach.
  5. Coordinate with RMYA Maintenance Team to ensure vehicle safety and maintenance. Ensure sanitation of vehicle occurs daily.
  6. Coordinate Volunteers to assist with outreach activities.
  7. Thoroughly understand and explain RMYA’s admissions procedures, clients we serve, and any required documents.

Relationships and Collaboration:

  1. Works in collaboration with program and all other agency staff to facilitate a team environment; participates in team meetings and committing to group decisions; role models effective team behaviors; demonstrates clear communication skills in building relationships with all employees and clients.
  2. Creates good working relationships with local youth servicing agencies and other area service providers and support groups to facilitate access to area resources for the clients.
  3. Maintain professional and ethical standards as prescribed by RMYA, and/or held license.
  4. Maintain personally required training hours.
  5. Maintain confidentiality in all areas of client and program operations.
  6. Represent RMYA in a professional manner at approved events in the community.
  7. Be supportive of all RMYA policies and procedures, and actively participate in their development by proposing changes and updates as well as working on teams designed to improve how we work together.
  8. Perform special projects as assigned.

Qualifications


Required Qualifications:

  • Education and Experience:
    • Bachelor’s degree in social science or related field and 1 year case management experience preferred, or
    • Master’s degree in social science or related field
  • Certification/Licenses: Valid Texas driver’s license or ability to obtain within 90 days of hire. Must have and maintain acceptable driving record.

  • Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Constant walking, standing, sitting, bending, arm and wrist rotation, reaching, writing, typing; frequent lifting and carrying at least ten pounds, pushing, pulling, and occasional driving.
  • Work Environment: Willingness to work flexible hours, including evenings, weekends, and holidays. Shifts in environmental conditions i.e., hot, cold, outdoor, indoor, dry, humid, noise, congested areas. fast-paced work using sensory demands i.e., hearing, vision, speech, spatial perception; dealing with multiple, complex, and repetitive tasks, working under pressure and deadlines. May be requested to work overtime and weekends for special program events. 

Preferred Qualifications:

  • Education: Master’s degree in social work or related field preferred.
  • Experience: Previous experience in an emergency client-service environment with children and adolescents preferred.

    • Business fundamentals- knowledge of basic business principles, trends, and economics.

Work Authorization/Security Clearance Requirements:

Successful clearance of Department of Family and Protective Services licensing background checks, (to include fingerprinting and drug testing) and maintenance of status providing ability to be present at an operation.
