Job Overview
As a compassionate and dedicated Licensed Practical Nurse, you will play a crucial role in providing exceptional resident care, supporting medical staff, and ensuring that residents receive optimal treatment in a variety of settings, including long-term care, sub-acute care, and memory care. We are looking for someone with strong clinical skills, excellent communication abilities, and a commitment to resident-centered care.
Responsibilities Include :
Flexibility to work in multiple facility locations, including North Reading, Lawrence, Braintree, and Wayland.Availability to work a minimum of 4 weekend shifts each month.Assess resident health issues and needs, creating and implementing effective nursing care plans.Administer medications and treatments as prescribed by physicians.Monitor resident progress and respond to any changes in their condition.Collaborate with multidisciplinary teams to deliver comprehensive care to residents.Educate residents and their families about health conditions, treatment plans, and discharge procedures.Maintain accurate resident records and documentation in line with state and federal regulations, ensuring HIPAA compliance.Participate in case management activities to promote continuity of care.Provide support in specialized services such as physical therapy, occupational therapy, substance use disorder management, and pain management.Required Skills :
Current Licensed Practical Nurse license in the state of Massachusetts.Experience with medication administration.Familiarity with EMR systems, particularly PCC (Point Click Care).Proficient knowledge of memory care techniques and protocols.Understanding of MDS (Minimum Data Set) assessments for long-term care facilities.Awareness of operations within Skilled Nursing, Sub-Acute, and Dementia Units.Strong background in case management practices.Ability to work effectively in a medical office environment.Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.Strong organizational skills to manage multiple tasks efficiently.Benefits :
