35-40 / hr Mileage Reimbursement

POSITION SUMMARY :
The Clinical Care Coordinator is a nurse who engages incommunity outreach to counsel, explain and educate patients and families abouthospice and palliative care services, reviews consents and other documents andassists with election of hospice benefit. The Clinical Care Coordinatorcoordinates patient discharge from hospital into hospice or palliative careprograms and performs start of care and initial nursing assessment. TheClinical Care Coordinator works cooperatively in coordination with the hospiceand / or palliative care teams. TheClinical Care Coordinator requires desire to work directly with patients andfamilies to assist them efficiently accessing hospice and / or palliative careservices. Must be able to utilize computers and becomfortable with electronic medical record. Must be able to meet with patients andfamilies in person and / or remotely
