Roswell Park Care Network is looking to hire an Advanced Practice Practitioner for our beautiful Orchard Park, NY location.
The Advanced Practice Practitioner provides healthcare services to patients including health promotion, cancer prevention, care for acute and chronic phases of illness, as well as patient and family education and counseling. This role also refers patients to other healthcare providers and community resources when appropriate.
Brief Description of Duties:
- Assesses the physical and psychological status of patients by means of interview, health history, physical examination, and diagnostic studies.
- Recognizes deviations from normal in physical assessment; works in collaboration with physician in formulating treatment plans for health problems and follow-up.
- Writes prescriptions for medications and blood products based on laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post discharge care in accordance with written practice protocols.
- Review’s findings and determines eligibility for entry into clinical trials.
- Collaborates with other members of the healthcare team, requests written consultation from physicians and other healthcare professionals as appropriate.
- Interprets and evaluates findings from diagnostic studies.
- Relays appropriate actions to facilitate and implement therapeutic plans consistent with the continuing care needs of the patient.
- Administers therapeutic measures/obtains specimens as appropriate.
- Evaluates the quality of care provided and recommends changes for improvement in the delivery of care.
- Initiates appropriate actions to facilitate the implementation of therapeutic plans consistent with the continuing care needs of the patient.
- Dictates or writes patient history, admission notes, care plans, progress notes, discharge notes and follow-up letters.
- Assists in management of family dynamics and coping mechanisms during the acute and chronic phase of the patient’s care.
- Triage’s patients appropriately and provides emergency treatment as necessary.
License to practice as a Registered Nurse in the State of New York- Current Nurse Practitioner Certification in New York
- Certification or eligibility for certification as a Nurse Practitioner by the American Nurses Credentialing Center or other national certifying body recognized by the Health Care Financing Administration (HCFA).
OR
- Bachelors degree in Physician Assistant studies; or
- Bachelor's degree and completion of a Physician Assistant Certification Program recognized by the New York State Education Department.
- Physician Assistant Certification to Practice in New York State
- 6 months experience preferred.
- Certification Basic Life Support (BLS).
