Title: LPN, LVN, or LCSW

Location: REMOTE – San Antonio, TX

Duration: 6* Months Contract*

Start Date: 04/21/2025

Shift: 8:00 AM - 5:00 PM CST

Position Purpose:

• Supports administrative care management activities including performing outreach, answering inbound calls, and scheduling services. Serves as a point of contact to members, providers, and staff to resolve issues and document member records following current state and regulatory guidelines.
• Provides outreach to members via phone to support them with care plan next steps, community or health plan resources, questions or concerns related to scheduling, and ongoing education for both the member and provider throughout care/service
• Provides support to members to connect them to other health plans and community resources to ensure they are receiving high-quality customer care/service
• May apply working knowledge of assigned health plan(s) activities and resources
• Serves as the front-line support on various member and/or provider inquiries, requests, or concerns which may include explaining care plan procedures, and protocols
• Supports member onboarding and day-to-day administrative duties including sending out welcome letters, related correspondence, and program educational materials to assist in the facilitation of a successful member/provider relationship
• Documents and maintains non-clinical member records to ensure standards of practice and policies are following state and regulatory requirements and provide to providers as needed

Required Skills/Experience/Education:

• LPN, LVN, or LCSW Required
• Experience working with disabilities/vulnerable populations who have chronic or complex conditions.
• Top 3 Must-have hard skills: Phone Customer Service, Computer Skills & Proficient with Microsoft Office (Word, Excel, Outlook), SharePoint, Internet
• Professional Spoken Communication Skills, Typing Skills (40 WPM), and multitasking using multiple systems and programs simultaneously.
• A reliable home internet is required (a hard-wired internet connection) with a dedicated workspace due to HIPAA.
• Proof of High School Diploma/GED or higher is REQUIRED. Must have an on-hand copy of HSD/GED/Transcript. A bachelor's in social work, Health Services, or Behavioral Science, is preferred but not required.

Job Type: Contract

Pay: $26.00 per hour
