Position Summary:
The Case Manager is responsible for providing supportive case management services to clients receiving counseling and behavioral health services. This role involves assessing client needs, coordinating care, linking clients to community resources, advocating for services, and supporting clients in achieving their recovery or treatment goals. The Case Manager works as part of a multidisciplinary team to ensure comprehensive, client-centered care.
Key Responsibilities:
Conduct initial intake paperwork
Coordinate referrals to internal and external services, such as housing, employment, healthcare, substance use treatment, and social services.
Monitor client progress, update service plans regularly, and adjust support as needed.
Collaborate with therapists, medical providers, peer support specialists, and other professionals involved in the client's care.
Advocate on behalf of clients to ensure access to needed services and benefits.
Maintain accurate and timely documentation in accordance with agency and funder requirements.
Participate in team meetings, trainings, and case consultations.
Provide crisis intervention support or assist clients in accessing emergency services when appropriate.
Support client transitions (e.g., from inpatient to outpatient care, reentry from incarceration, or discharge planning).
Answering phones and scheduling appointments
Collecting specimens for drug screening
Qualifications:
Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a related field preferred
Experience: At least 1 year of experience in case management, social services, or behavioral health preferred.

Skills:
Strong organizational and communication skills
Ability to build rapport and maintain professional boundaries
Knowledge of community resources and support systems
Comfort working with diverse populations and individuals with complex needs
Ability to work independently and as part of a team
Other: Valid driver’s license and reliable transportation (if fieldwork is required). Background check and clearances required per agency policy.
Preferred Qualifications:
Experience with electronic health record (EHR) systems
