  • Position Summary :

Under the direction of the Chief Medical Officer and the supervision of a Physician, the Nurse Practitioner works as part of the medical provider team, providing medical services to the patients of the community health center. Nurse Practitioners provide primary care services as licensed independent practitioners in accordance with State and Federal rules and regulations and SC Board of Nursing Standards of Care. The Nurse Practitioner will actively participate in the implementation and evaluation of the Patient Centered Medical Home using team based care to achieve improved health outcomes as well as improved patient experiences.

II. Essential Functions / Responsibilities :

  • Develops rapport with patients striving to build a provider-patient partnership that facilitates marked familiarity with patients medical and treatment history, care needs, and psychosocial factors as a means to manage patient’s risk factors and improve patient outcomes.
  • Provides primary medical care by caring for all patients in accordance with the practitioner’s medical specialty.
  • Develops a plan of care for each patient including : complete medical history, physical examination, diagnosis, appropriate treatment and / or referral, including hospitalization where necessary.
  • Stresses the importance of preventive health care measures.
  • Confers with supervising physician and consulting physicians, nurse, patient, and patient’s family concerning treatment and care of patient.
  • Establishes an effective working relationship with public and private physicians and other clinical referral resources.
  • Attends all medical staff meetings and participates in the community health center’s Q / A program.
  • Performs other necessary duties as required by the community health center to meet the goals of providing primary health care.
  • Participates in the clinical program of the organization according to the guidelines established for all clinical staff of similar training and board certification.
  • Uses all available resources in diagnosis and treatment, such as laboratory and radiological testing, in an appropriate and cost efficient manner.
  • Refers those cases which require specialist services, but maintains responsibility, assuring that continuity of care if provided.
  • Completes all records and reports as assigned.
  • Participates in the development and implementation of a regularly updated quality assurance plan for the organization.
  • Participates in the Health Promotion / Disease Prevention activities required by the U. S. Public Health Service.
  • Ensures an appropriate environment for the administration of healthcare by keeping all equipment and work areas in a clean, safe, and orderly fashion.
  • In performing job duties, may be exposed to blood and body fluids, hazardous chemicals, operating technical equipment and prolonged standing
  • Willing to travel in Aiken and surrounding counties to provide services for patients across the lifespan (infancy to adult).

    Provider champion for Early Childhood Development Program, School based program, and provide services to adult Agriculture workers.

    Comfortable with working in a more confine space / mobile unit while providing medical services to clients in the community.

    Flexible to assist in providing medical services to students and adults in designated area within selected schools.

    Mileage reimbursement available.

    Full time position with early mornings to accommodate school schedules.

  • Ability to comprehend, interpret and apply basic laws and regulations to specific situations.
  • Ability to assist in the development of department administrative directives as required.
  • Ability to assist in investigative analysis and make effective recommendations as required.
  • Ability to prepare clear, sound, accurate and informative reports containing findings, conclusions and recommendations as required.
  • Ability to prepare clear, accurate and informative reports of progress of organizational units, results of research and reviews, and annual reports of accomplishments, future goals, and objectives.
  • Ability to work effectively with associates, as well as with supervisors, in the various units of the organization.
  • Ability to work in a team relationship.
  • Experience / Education Required :

  • Graduate of an accredited School of Nursing.
  • Completion of an accredited Nurse Practitioner program.
  • Current Advanced Practice Registered Nurse (APRN) Licensure
  • Current Nurse Practitioner certification in specialty (family preferred)
  • Current provider CPR
  • Two (2) years experience preferred.
  • Current MAT certification or willingness to obtain MAT certification
