You may be an excellent candidate for the Service Coordinator I position working as a member of the Child and Family Connections #11 program at Rush University Medical Center.

The Family Care Coordinator works with families located in the Near North neighborhoods of Chicago, up to and including Evanston. The Child and Family Connections office is located off campus in the Near North Side of Chicago.

This is a Full Time position
80 hours every two weeks
Monday to Friday
40% of time in office, 60% of time remote

Position Highlights:

The Service Coordinator I with Child and Family Connections assists in determining eligibility for designated services and enables families to receive their rights, procedural safeguards, and services that are authorized under the state's early intervention program. The Service Coordinator builds relationships that include providing information to families and coordinating information among multiple service providers. The service coordinator maximizes the use of existing resources and refers families to any other needed services, tailoring the unique circumstances of each individual child and family. The Service Coordinator exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Position Responsibilities:

Provides direction to families on the early intervention system in the state and local area, including their rights and availability of services.
Assists families in identifying available service options, including the availability of advocacy services.
Gathers all pertinent medical, developmental and social information for the child and family and coordinates the performance of evaluations and assessments in a timely manner.
Facilitates and participates in the development, review, evaluation and maintenance of Individualized Family Service Plans (IFSP).
Coordinates and monitors the ongoing delivery of services to ensure that all resources are being utilized and the IFSP goals are being addressed.
Gathers all pertinent family insurance and income documentation and assists families in understanding fee and insurance usage in the early intervention system.
Maintains the child's CFC record in a clear, complete format following DHS and Rush policy, including correspondence and case notes.
Facilitates the development and implementation of transition plans, including school or other services as appropriate.
Communicates effectively within the hospital and department, as well as with families, community providers and the Department of Human Services.
Responsible for personal and professional growth and development.

Position Qualifications Include:

Bachelor's degree in a related human service field such as early childhood special education, social work or psychology.
Must meet requirements to obtain Illinois Department of Human Services Part C Early Intervention Service Coordination Credential.
Successful completion of state-sponsored Service Coordinator, Systems Overview, and Cornerstone trainings are required.
Ability to gather information, format reports, and present outcome of work assigned.
Interpersonal skills are integral to the position and the ability to coordinate activities with many disciplines toward one goal is essential.
Case management experience is essential.
Good time management skills and the ability to work independently are required.
Demonstrates knowledge of the developmental, physical, psychosocial, medical and cultural needs of children and families.
Demonstrates excellent organizational and case management abilities.
Ability to work with different cultures and aid family with various barriers to care is essential.
Physical: able to be active and travel. Use of personal car required.
Mental: able to understand complex diagnosis and treatment information and use deductive reasoning.
Social: collaborates and supports the objectives of the Child and Family Connections grant.
Knowledge of the developmental, physical, psychosocial, and cognitive development of children 0-3 years is preferred.
Basic knowledge of typical/atypical child development is preferred.
