SUMMARY:
The Community-Based Therapist is a professional position providing clinical therapy services to children and families in the community. Community spaces include the client’s home, school, and other facilities clients spend time in, or other facilities that are related to the client’s treatment. The position is also responsible for providing case management support services to children that meet the SED criteria and their families, per Medicaid guidelines. The Community-Based therapist provides individual, family, and group therapy, and coordination of services with other entities such as MDHHS, juvenile justice, or school systems.
WHAT YOU'LL DO

- Conduct comprehensive biopsychosocial assessments with clients and families, either in-person or virtually.
- Complete person-centered treatment plans with clients and families and evaluate progress on an on-going basis.
- Write gender-affirming medical treatment letters such as letters for Hormone Replacement Therapy (HRT) and gender-affirming surgery.
- Maintain timely and accurate client records to ensure they meet DWIHN (Detroit Wayne Integrated Health Network), CARF, and agency standards.
- Maintain a caseload of clients in accordance with MDHHS standards and Ruth Ellis Center policy.
- Provide individual, family, and group therapy in community settings.
- Utilizes best practices for serving the LGBTQ2S community including evidence-based practices, trauma informed practices, substance use treatment, crisis intervention, and more.
- Provides referrals and recommendations for supportive services as needed.
- Provide Case Management services as needed for clients and families.
- Collaborate with entities such as Juvenile Justice, foster care, MDHHS, community management organizations, other community partners and natural supports involved in the client’s care.
- Maintains confidentiality of records relating to clients' treatment according to HIPAA standards.
- Working knowledge of the DSM-5 and diagnostic work
- Participate in weekly, bi-weekly, and monthly team and staff meetings.
- Participate in case consultations with an Integrated Care Team.
- Prepare required reports for MDHHS, courts, schools, etc.
- Responsible for duties assigned related to grant funded projects.
- Shares Community Mental Health Program information widely (schools, meetings, stakeholders) with intent to recruit clientele into appropriate services
WHAT YOU'LL NEED:
1. A Master's degree from an accredited college or university with a major in Social Work, Psychology, Counseling, or related field and a limited or full license as an LMSW, LPC, or LP by the state of Michigan.
2. Demonstrated experience with general mental health diagnosis and treatment.
3. Experience working with individuals and families involved with Department of Health and Human Services, Foster Care, Juvenile Justice, and experience with homelessness.
4. Demonstrated knowledge of adolescent development and positive LGBTQ development.
5. Experience with Microsoft Office, and Google Suite Software.
