What are the responsibilities and job description for the CHT Social Worker position at Rutland Regional Medical Center?
The CHT Social Worker is a VT Blueprint for Health funded position responsible for providing social work case management, care coordination, transitional care interventions and community resource referral for patients in NCQA recognized Medical Homes who are referred to the Community Health Team, primarily those with chronic conditions. This work entails individual patient case management and collaboration with other health care professionals both in the community and as part of the Core Community Health Team. The CHT Social Worker understands community resources, insurance, regulatory and legal system issues relevant to the transition of patients from inpatient and emergency department to community settings.
Minimum Education
- Master’s Degree in Social Work.
Minimum Work Experience
- 3 years of LICSW social work and medical crisis intervention, solution-focused therapy, and knowledge of community resources or related experience.
- Experience working with adults with acute mental illness and substance abuse disorders.
- Managed-care experience a plus.
Required Licenses/Certifications
- Licensed by the State of Vermont as a Clinical Social Worker (LICSW)*
- or evidence of obtaining within one year.

- Psychosocial assessments, utilization and case management principles.
- Individual and family supportive counseling and advocacy.
- Age-specific competencies.
- Interdisciplinary collaboration.
- Familiarity with regulatory standards relevant to acute inpatient and outpatient psychological services.
- Verbal and written communication skills.
- Basic Microsoft Windows desktop application and navigation skills.
Salary Range: $28.99 - $46.29
