Responsibilities:
- Provide skilled nursing care to patients in their homes
- Develop and implement individualized care plans
- Monitor patient conditions and communicate with the care team
- Educate patients and families on health management and medications
- Document all care and communication accurately and timely
Qualifications:
- Valid RN license in the state of California
- Minimum of 1 year of skilled Home health experience (Home Health experience preferred)
- Current CPR certification
- Reliable transportation and valid driver's license
- Excellent communication and organizational skills
