What are the responsibilities and job description for the Hospice RN Case Manager (Southern Fairfield County) position at RVNAhealth?
We are currently seeking a Hospice Registered Nurse Case Manager. This position will cover Southern Fairfield County (Stamford/Greenwich).
As a Hospice RN, your mission will be to deliver compassionate and personalized patient care in alignment with physician's orders. Your responsibilities will include:
- Regularly assesses and reassesses the nursing needs of the Hospice patient. Competently implements care needed for comfort and palliation of symptoms
- Provides direct client care and works closely with case manager in implementing care.
- Performs admission procedure as per RVNAHealth policies using electronic health record in a timely manner with accurate documentation. Develops a plan of care consistent with the physician's plan of treatment and patient/ family wishes.
- Coordinates the implementation of the plan of care for patients residing in SNF, ALF, hospital contract bed or personal residence.
- Provides dietary counseling.
- Provides Hospice nursing services, treatments and preventive procedures.
- Initiates nursing procedures appropriate for the patient's Hospice care and safety.
- Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient's physical or emotional condition.
- Teaches, supervises and counsels the Hospice patient, family members and facility staff about providing care for the client.
- Graduation from an accredited school of nursing; BSN preferred
- Currently licensed to practice as a Registered Nurse in the State of Connecticut.
- A minimum of two years of nursing experience, preferably in medical/surgical, oncology , Critical Care, ED or labor and delivery nursing. Home Health Care and/or Hospice experience preferred. Active patient contact within past three years, preferred.

Compensation:
Our target pay range for this position is $96,500 to $106,500 per year. This pay range represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
