What are the responsibilities and job description for the RN Hospice On Call (South) position at RVNAhealth?
The On Call Hospice RN is responsible for ensuring that patient care is coordinated and managed appropriately during hours that the office is closed through triaging issues via phone or making visits. In general, responsible for evening, night, weekend, and holiday on-call duty as scheduled. Expected hours for this full time on-call position are 4:30pm-8:30am position. Will rotate with another RN throughout the week and will cover the Southern areas/towns of Fairfield County, CT.
Responsibilities:
• Provides skilled nursing care to hospice clients in their homes or community health settings ensuring the delivery of outcome based, high quality, cost effective care.
• Regularly assesses and reassesses the nursing needs of the Hospice client.
• Provides direct client care.
• Performs evaluation of clients' progress, promptly acts on any changes made to client's plan of treatment or condition.
• Executes the plan of care for clients residing in SNF, ALF, hospital contract bed or personal residence.
• Provides Hospice nursing services, treatments and preventive procedures.
• Teaches, supervises and counsels the Hospice client and family members about providing care for the client.
• Documents communication and interprets plan of care to the client regarding hospice diagnosis and symptom management.
• Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor's orders.

Qualifications:
- Graduate of an accredited school of nursing; BSN preferred
- Currently licensed as a Registered Nurse in the State of Connecticut.
- A minimum of two years of clinical experience
- Preferred, minimum one year of hospice or home care experience with a strong knowledge base of community care and associated regulations.
- Active client contact within past three years, preferred.

Compensation:
Our target pay range for this position is $92,000 to $106,500 per year. This pay range represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
