What are the responsibilities and job description for the Triage RN position at RVNAhealth?
The Extended Hours Triage Registered Nurse is responsible for ensuring that exceptional, quality, person-centered care is provided by the Hospice team during extended hours.
This position will be involved with all aspects of clinical operations, including ensuring Homecare and Hospice Benefits (HCHB) workflow is completed on a timely basis so patient care is not delayed, working with clinicians in the field to provide guidance in patient care.
The individual in this position will collaborate with referral sources, patients, families, field clinicians, and assist the Hospice Manager in leadership of the field clinicians during extended hours.
Schedule:
Saturday/Sunday - 8:30am - 4:30pm - EVERY OTHER WEEKEND
Triage Registered Nurse Responsibilities:
- Responsible for triage of inbound patient calls and coordinating visits with field clinicians as necessary.
- Possess and maintain a thorough grasp of clinical and regulatory knowledge pertaining to various disease states to ensure care is the right care for the right time of trajectory of disease.
- Responsible for the management of referrals for hospice service lines during extended hours.
- Responsible for the oversight and task assignment to Team Coordinators during Extended Hours.
- Perform clinical workflow Hospice Service Lines throughout Extended Hours to ensure patient care is timely.
- Responsible for extended hours triage, scheduling, and referral management of hospice service lines.
- Perform hospice clinical workflow process tasks in Electronic Medical Records (EMR) (HomeCare HomeBase) to ensure timely completion of tasks so patient care can be completed quickly and within any necessary patient needs, regulatory, payor, or other timelines.
- Perform other clinical operation tasks or projects as assigned by Hospice Branch Manager.
- Responsible for initial push of Standard Operating Codes (SOC's) to the calendar in concert with Hospice Branch Manager.
Qualifications:
- Graduation from an accredited program in Nursing.
- Current license to practice as a Registered Nurse in Connecticut.
- Hospice experience required.
- Computer skills.
- Ability to identify needs in workflow process.
- Decision Maker, able to multi-task.
- Knowledgeable and compliant with all agency and regulatory procedures, including but not limited to Corporate Compliance, Health Insurance Portability and Accountability Act (HIPPA), Medicare conditions of participation, State of CT Licensure, and Commission on Cancer Accreditation (CHAP) Standards.
