Job Overview:
The Hospital Care Coordinator plays a vital role in the value-based operations care management team, overseeing care coordination for patients enrolled in value-based programs during their stay in an acute care setting.
For Medicare patients in the Medicare Shared Savings Program (MSSP) and the Hospital Readmissions Reduction Program (HRRP), the Care Coordinator engages in interdisciplinary rounds and collaborates with the care team to determine discharge plans. For other contracted payers, the Coordinator will coordinate similarly with the hospital team.
Qualifications:
Required:
Associate's Degree in a healthcare-related field
Knowledge of healthcare economics and regulations (skilled nursing, home health, BPCI A, MSSP/ACOs, CMS Waiver Programs).
Preferred:
- At least one year Case Management experience, preferred.
- Current Active NJ RN or LPN License, preferred.
Essential Job Functions
Identify patients under MSSP and HRRP upon admission through hospital programs in Epic.
Document patient data in Epic Healthy Planet, focusing on discharge details (next site of care)
Engage with care management leadership and the hospital team to optimize patient care.
Communicate with discharge planners regarding post-acute care needs and coordinate follow-up with care managers.
Conduct follow-up calls after discharge to ensure safe transitions and address any concerns.
Report readmissions to Case Management.
Assist patients with MyChart sign-ups and scheduling follow-up appointments with primary care providers or specialists.
