Qualifications: is a (1) graduate of a college or university with a degree in a related human service field and has two (2) year of full-time, post-graduate accumulated MH/DD/SAS experience with the population served or; (2) a graduate of a college or university with a baccalaureate degree in a not related human service field and has two (4) years of full -time, post-baccalaureate accumulated MH/DD/SAS experience with the population served.; and/or (3) a Registered nurse who is licensed to practice in the State of NC by the NC Board of Nursing and has four (4) years of full-time accumulated experience in psychiatric mental health nursing.
Reports to: Executive Director
Job Duties:
§ Assist in assessing member strengths and needs.
§ Provide Management and oversight of caseload members and caregivers assigned.
§ Assist in program planning.
§ Perform quality assurance checks and documentation assuring such as medical record guidelines are followed and specific program documentation are followed.
§ Supervises service provision assuring such as all services and programs are delivered as specified in the plan, progress is recorded accurately and immediately, all member rights policies are followed, and member is treated with dignity and respect.
§ Assist care provider with evaluating progress.
§ Develop task analyses as identified by the MCO.
§ Serve as substitute care provider as deemed necessary in emergency situations.
§ Serve as primary liaison with Care Manager in MCOs served.
§ Provide quarterly written progress report to Care Manager on each member served.
§ Maintain regular contact with member, care provider, families and other professionals supporting each member.
§ Provide orientation and ongoing trainings to care providers.
§ Provide exceptional customer service and build relationships with families.
§ Ensure compliance responsibilities are maintained at all times.

Pay: $48,019.00 - $55,405.00 per year
