JOB SUMMARY :
Provides quality restorative care to assist patients / residents to achieve / maintain optimum level of function in conjunction with goals identified on restorative plan of care
QUALIFICATIONS :
1. Minimum high school diploma or GED preferred.
2. Certified as a CNA in the state of Illinois / Indiana or able to be certified within 120 days.
3. Certified in CPR.
4. Ability to express or exchange spoken and / or written ideas in English with patient / residents, co-workers, families, and the public.
5. Ability to learn and use current technology.
6. One-year experience in long-term care preferred.
7. Knowledge of restorative nursing assistant functions or must complete restorative course within one year of hire.
PHYSICAL REQUIREMENTS :
1. Frequently and safely lifts and transfers patients / residents weighing an average of 150 pounds, with designated lifting equipment and / or safe lifting techniques.
2. Transports patients / residents in wheelchairs and other devices to various areas in the ministry.
3. Frequently twists, stoops, bends, crouches, kneels, and reaches above or below shoulder height.
4. Ability to work a full shift on one's feet.
5. Frequently lifts and carries equipment and supplies weighing 25 pounds.
6. Must be able to express or exchange spoken and / or written ideas in English with patient / residents, co-workers, families, and the public.
REPORTS TO : Restorative Nurse / Nursing Administration
ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING :
1. Provides input to the Restorative Nurse for the development of the restorative care program.
2. Assists with Implementation of nursing restorative interventions as directed by restorative nurse including but not limited to ambulation, range of motion (ROM), activities of daily living (AOL), assistive feeding devices, splints, hand cones / contracture care, and bladder retraining.
3. Participates in care plan conferences as directed.
4. Completes restorative documentation related to individual resident's participation, progress, tolerance, etc.
5. Works with appropriate departments to ensure assistive devices are consistently available and utilized on assigned patients / residents.
6. Trains and monitors other ministry staff on use of patient / resident specific assistive devices and transfer equipment.
7. Assists restorative nurse in providing staff education and competencies related to restorative care.
8. Reports observations and other pertinent information related to care of patients / residents to appropriate staff.
9. Performs other duties as assigned by the Restorative Nurse, ADON or DON.
GENERAL REQUIREMENTS :
1. Demonstrate critical thinking skills, including the ability to synthesize information and make role-appropriate decisions.
2. Requires regular predictable attendance and punctuality.
3. Adheres to Federal and State regulations governing long-term care.
4. Maintains a neat, well-groomed appearance and adheres to the dress code of the position.
5. Must be knowledgeable of ministry fire and safety procedures.
6. Must be knowledgeable of emergency procedures such as tornado, power failure, etc.
7. Must be knowledgeable of appropriate infection control practices.
8. Attends mandatory in-services.
