Position Overview
-
The Licensed Practical Nurse (LPN) provides direct patient care in accordance with Illinois state
-
laws and regulations. The LPN will work under the supervision of a Registered Nurse (RN) or
-
physician to deliver high-quality nursing care, support patient needs, and ensure compliance with
-
healthcare standards and regulations.

Key Responsibilities
-
1. Patient Care
-
o Administer medications and treatments as prescribed by a physician or nurse
-
practitioner.
-
o Monitor and record vital signs, including temperature, pulse, respiration, and
-
blood pressure.
-
o Assist with patient assessments, including identifying changes in patient
-
conditions and reporting them to the RN or physician.
-
o Provide wound care and assist with the insertion of catheters, including
-
monitoring for complications.

2. Clinical Duties
-
o Perform clinical procedures such as dressing changes, injections, and specimen
-
collections according to protocols.
-
o Document patient care and progress accurately and comprehensively in medical
-
records.
-
o Assist with developing and implementing patient care plans in collaboration with
-
the healthcare team.

3. Communication and Collaboration
-
o Communicate effectively with patients, families, and healthcare team members to
-
provide updates and address concerns.

o Collaborate with RNs and physicians to coordinate patient care and ensure
-
adherence to care plans.

o Provide patient education on health management, including medication
-
instructions and discharge planning.

4. Infection Control and Safety
-
o Follow infection control practices, including proper hand hygiene and the use of
-
personal protective equipment (PPE).
-
o Adhere to safety protocols to prevent patient falls, medication errors, and other
-
safety hazards.

o Participate in ongoing training and education on infection control and safety
-
procedures.

5. Compliance and Professional Conduct

o Adhere to Illinois state nursing regulations and laws, including maintaining an

active LPN license.

o Follow facility policies and procedures related to patient care, safety, and ethical

practice.

o Attend and participate in required training, orientation, and team meetings.

Qualifications
-
Education and Certification:
-
o Completion of an accredited LPN/LVN program.
-
o Current licensure as an LPN in Illinois.
-
o Current Basic Life Support (BLS) certification preferred.

Experience:

o Previous experience in a healthcare setting preferred but not required.

o Demonstrated ability to provide effective and compassionate patient care.

Skills and Abilities:

o Ability to work independently and as part of a healthcare team.
