Position Title: Supervisory Nurse (RN/LPN)
Reports To: Director of Nursing / Agency Administrator
Job Summary:
The Supervisory Nurse oversees home health aides and homemakers, ensuring they
provide quality care in accordance with Medicaid guidelines, state regulations, and
agency policies. The nurse conducts initial assessments, periodic supervisory visits,
and compliance reviews.
Essential Duties & Responsibilities:
Client Assessments & Care Plans:
o Conduct initial assessments for new clients and develop a personalized
care plan.
o Review and update care plans every 60-90 days or as needed.
o Monitor clients for changes in condition and adjust care plans
accordingly.
Supervision & Training:
o Perform supervisory visits every 60 days for home health aides per
Medicaid requirements.
o Provide training and performance feedback to aides.
o Ensure aides follow infection control, safety, and HIPAA regulations.
Compliance & Documentation:
o Review EVV records, aide task sheets, and client progress notes.
o Ensure compliance with Ohio Medicaid (5160-32) and Section 121.36 for
audits.
o Assist with corrective action plans if compliance issues arise.
Communication & Coordination:
o Act as the primary liaison between clients, caregivers, case managers,
and physicians.
o Address client concerns and service adjustments as needed.
o Ensure emergency procedures and protocols are followed.
Qualifications & Requirements:
Education: Associate’s or Bachelor’s degree in Nursing (ASN or BSN)
License:
o Active RN or LPN license in Ohio (must be in good standing)
Experience:
o At least 1 year of clinical nursing experience (home health experience
preferred)
o Prior experience in care coordination, compliance, or training is a plus
Skills & Abilities:
o Strong leadership, organizational, and problem-solving skills
o Knowledge of Medicaid policies, EVV, and documentation requirements
o Ability to travel locally for home visits and supervisory checks
Background Check & Compliance:
o Must pass FBI/BCI background check
o Valid driver’s license and reliable transportation
o Ability to work independently and manage multiple cases
