As a Hospice Liaison with St. Johns Hospice, you'll be responsible for building strong relationships with patients and their families as they navigate through difficult times. On a typical day, you can expect to connect with potential clients and provide them with information about our services. You'll collaborate with our team to assess patient needs and develop personalized care plans. Expect a fast-paced and dynamic environment with problem -solving and empathy are key. This role requires daily travel to build relationships and facilitate referrals, so a valid driver's license and reliable transportation are necessary. Bilingual skills are a plus!
