Saad Hospice is seeking a full-time LPN for our Field Hospice. The LPN typically work Monday-Friday 8 am to 5 pm with a call rotation. DUTIES AND RESPONSIBILITIES:
Maintain the standards of nursing care and follow the policies and procedures of the Hospice Agency.- Make PRN visits on our Field Hospice Patients.
- Assess the patient's condition and nursing needs; prescribe appropriate actions under the direction of the physician and hospice senior leadership.
- Assess pain and symptoms and also maintain pain and symptom control.
- Assess the nursing needs of the terminally ill patient.
- Interact with patients and family in maintaining physical, mental, and emotional support.
- Identify specific needs within the care setting and refer to the appropriate team member.
- Provide patient and family teaching regarding pain and symptom management, medications, dietary needs, and progression of disease process and the expected outcomes.
- Support and inform patients and family regarding expectations and Hospice concept, which allows them to make an informed decision.
- Ensure the patient's desires and rights are maintained.
- Whenever possible, actively support the patient and family during the dying process.
- Record observations timely, accurately, and concisely in accordance with Saad Hospice Documentation Standards.
- Participate as a team member in the interdisciplinary team meeting for identification of problems and problem solving.
- Maintain documentation within the medical records as required by state, federal, and agency policies in a timely manner in accordance with Saad Hospice Documentation Standards.
- Participate in the call rotation with the other staff members as deemed necessary.
