Key Responsibilities : Direct Patient Care : Provide hands-on physical therapy services as outlined by our agency and certified by the patient’s physician. Comprehensive Assessments : Evaluate, document, and communicate patients’ progress and status to the physician and healthcare team. Education & Supervision : Teach and guide nurses, home health aides, family members, and patients to ensure a continuous and effective physical therapy program. Standards of Care : Deliver therapy services in line with accepted standards for each patient’s condition. Care Plan Development : Collaborate with the healthcare team and physician to develop personalized care plans. Detailed Documentation : Maintain accurate and timely clinical records for each patient, documenting limitations and progress. Therapeutic Treatments : Utilize therapeutic exercises, heat and cold modalities, muscle re-education, and biofeedback to treat patients. Diagnostic Testing : Conduct diagnostic and prognostic tests to assess muscle, nerve, joint, and functional abilities. Home Exercise Programs : Create and instruct patients and their families on home exercise programs. Timely Reporting : Complete initial evaluations, clinical visit reports, progress notes, and discharge summaries promptly. Supervision : Oversee and direct Licensed Physical Therapist Assistants (LPTAs) in executing the treatment plan. Interdisciplinary Collaboration : Participate in the Interdisciplinary Patient Conference Committee to ensure comprehensive patient care. Why Saad Healthcare?
