Our Street Medicine team visits and walks through pre-determined nieghborhoods and provide general assessments and care to those that are experiencing homelessness. The MA assists the NP by taking vitals and recording patient data. In the afternoon, the MA returns to our clinic and inputs collected data into our EMR system.
Duties and Responsibilities :
Walking on-foot to serve individuals experiencing homelessness, including walking to homeless encampmentsWelcomes patients and helps set up services for patient to be seen at encampment by a Street Medical Provider.Conduct follow-ups with patients via phone, encampment visits, and alternative sites where patient can be found.Works in collaboration with staff from partnering organizations to schedule patients for follow up appointments.Works together with the Medical Provider and staff from the partnering organizations to manage flow of patients and maintain clinic supplies.Sets up exam area prior to clinic sessions, ensuring all necessary supplies are stocked and readily available.Utilizes the electronic health records (EHR) database to document and review prior patient history, test results, related information, and performs data entry to record information about the current appointment.Ability to schedule patient appointments and encounter patients using the EMR Appointment Scheduling / Encounter Registrations.Assists providers with examinations, lab collection, and procedures.Checks patients' vital signs; conducts vision and hearing screenings; EKGs, and venipuncture; records test results in the patients' electronic health records.Provides patient discharge instructions to patients and helps coordinate other patient care needs (including coordinating transportation) as appropriate.Work closely with Street Medical Provider, Nursing Supervisor, Programs Manager and Coordinator and several members of the multi-disciplinary team.Provide client care that is trauma-informed, non-judgmental, and culturally sensitive.Performs other duties as assigned by clinicians or supervisor.Position Requirements :
Prior clinical experience in a health care setting of two year or more in a similar community clinic environment preferred.Minimum of a Certification for Medical Assistant from a California accredited school.BLS CPR Certification.Experience in homeless healthcare strongly preferred.Skills in performing venipuncture, audiometry, EKG tests and vision screening tests.Maintain a professional, friendly and courteous demeanor in the performance of duties.Communicates effectively and courteously. Address and calms potentially tense situations.Ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles.Ability to travel independently to encampments and back to Saban Community Clinic parking site when required; both walking and drivingSelf-starter, reliable and dependable.About Us :
