OVERVIEW: The Team Leader is responsible for the overall management and supervision of the home visitation team as well as monitoring program integrity, assessing risk, and ensuring implementation of the framework for the parent support modalities utilized by the Family Resource Centers. The Team Leader is responsible for tracking and maintaining the quality of services provided at the Family Resource Center as well as participating in the recruitment and selection of qualified staff.
DUTIES:
- Case Supervision
- Provide culturally responsive reflective supervision and support to a team of no more than six Home Visitors per the policies and expectations of the program.
- Conduct joint home visits with Home Visitors to provide support with case planning as needed.
- Meet with each Home Visitor weekly to provide individualized strength-based supervision for a minimum of two hours per week.
- Ensure that an initial risk assessment of families is completed and collaborate with site management for ongoing support with case planning as needed.
- Work with Home Visitors to develop individualized case plans in accordance with the modality utilized by the program, including the family and all other involved parties in the process.
- Assign referrals to Home Visitors who will implement the appropriate modality based on the unique needs of the family.
- Review participant intake documents following the Program Policies and Procedures Manual to ensure completeness and accuracy.
- Maintain an open door policy assuring that informal supervision is available for staff to process their feelings around working with overburdened communities.
- Administrative and Personnel Management
- Assist with personnel management including, interviewing, hiring, orienting, evaluating, disciplining and terminating.
- Process timekeeping records, time studies, mileage sheets and check requests as needed.
- Review case files for compliance, accuracy, completeness and quality assurance.
- Schedule, coordinate, and facilitate monthly Multi-Resource Team meetings.
- Coordinate and facilitate home visitation team meetings at least two times per month.
- Facilitate and participate in team meetings, consultations and supervision to further enhance the collaborative nature of the program.
- Complete tasks in a timely manner based on programmatic policies and procedures.
- With the support of the Program Manager, monitor contract milestones pertinent to the home visitation program and review program milestones in order to develop strategies to improve outcomes.
- Participate in the evaluation of program milestones and outcomes for the purposes of ongoing quality assurance.
- Ensure that staff are in compliance with Sacramento County, Federal, and State regulations and standards applicable to the guidelines of the program model.
- Ensure staff compliance and completion of all mandated trainings.
- Ensure all staff receive culturally relevant trainings to ensure culturally responsive services are provided to the families.
- Attend and participate in all Team Leader meetings.
- Attend Sacramento County meetings as required.
- Plan and coordinate trainings by external speakers/presenters as necessary.
LOCATION: 4343 Williamsbourgh Dr. Sacramento, CA 95823
SCHEDULE: Monday – Friday, 8:30 am – 5:00 pm (Occasional weekends and evenings)
SALARY: $26.96 - $30.33 per hour, to start
QUALIFICATIONS
Education and Experience: A Bachelor’s degree in any field is required. Two years of experience in the social services field is required.
Experience in leadership roles and responsibilities with an ability to showcase what tasks specifically indicate these skills is preferred. This may include, but is not limited to, having been in a lead role in committees, projects, meeting facilitations, etc.
Driving is an essential function of this job. As such, employees in this position will be required to furnish their own vehicle to be used in the course of employment. A valid driver’s license and minimum insurance coverage is also required. Must be at least 21 years old.

- Able to read, write, speak, and understand the English language.

- Able to assess the role of trauma, mental health, and/or substance abuse as potential parenting risk factors.

- Knowledge of local neighborhoods, communities and cultures in Sacramento County
- Knowledge of team, family and group dynamics.
- Knowledge of Microsoft Word, Excel, Outlook, and Power Point.
- Excellent written and verbal communication skills.
