OVERVIEW:  Home Visitor’s work with a multi-resource team to provide home and community based support services to families identified at risk for child abuse and neglect.

 

 

DUTIES:

  • Home Visitation
  • Conduct in-depth strengths, risks and needs analysis of all members of the family referred to the program.
  • Work with the team, under the direction of the Project Coordinator, to develop and implement an appropriate family nurturing plan for each participant in order to meet his/her needs/goals.
  • Provide home visitation services to families with children 0-5 commensurate with families’ needs and program policies. 
  • Assist families to engage in appropriate problem solving and coping skills.
  • Promote appropriate parenting and disciplinary skills with parents. Provide positive role modeling through appropriate verbal redirection of children.
  • Provide parents with age appropriate developmental activities and expectations for their children.
  • Staff all critical incidents with a supervisor in a timely manner to ensure safety in compliance with program policies.
  • Assist family members to better understand their responsibility for the health and well being of themselves and their children.
  • Utilize community resources to help families meet the goals on their family nurturing plan, referring families to appropriate community resources as needed.
  • Provide/Arrange transportation to appointments, meetings and classes as needed.
  • Advocate for families, children w/ Health Care Providers, Dept. Human Health Assistance and Schools.
  • Follow curriculum based materials to educate families.
  •    Parenting Workshops/Parent Support
  • Deliver parenting workshops, including planning, coordinating, and administering appropriate assessments to parents of children 0-5 years old.
  • Co-facilitate parenting workshops with Parent Leaders in the FRC and community. 
  • Assist in planning parent support groups, activities, and special events for the FRC and community in cooperation with the Project Coordinator.  Participate as requested.
  • Exhibit a nurturing attitude and behavior in all contact with program participants and teammates.
  • Link parents to appropriate community resources based on the family’s needs.
  • Advocate for and act as a liaison for families with existing community services and agencies; this may include routine translation and interpretation.
  • Assist families with self-advocacy, empowerment and constructive problem-solving as needed.
  • Conduct appropriate screening and assessments when appropriate to families utilizing the FRC.
  • Model appropriate parenting behavior, provide support and education/information/referrals to parents.
  • Provide parents with age appropriate parenting and positive discipline techniques.
  • Provide approved transportation in an agency vehicle to the FRC for parenting workshops, parenting activities, and/or events consistent with program standards.
  • Write and maintain accurate family case files, including but not limited to, correspondence, screenings, check lists, referrals, etc. 
  • Record all communication with families in case files.
  • Write progress notes in a timely manner.
  • Complete a Family Nurturing Plan with each family.
  • Complete assessments and review results with each family.
  • Prepare Critical incident and Suspected Child Abuse reports as necessary.
  • Prepare forms required for multi-resource team meeting presentations on all families.
  • Follow up with referrals to community-based resources.
  • Prepare for and actively participate in staff meetings, multi-resource team meetings, training, case review, etc. as required.
  • Prepare for and participate in weekly supervision as scheduled by the Project Coordinator.
  • Research additional resources and services for families.
  • Provide additional support to the Family Resource Center as needed.
  •  Outreach
  • Assist the Project Coordinator in developing and implementing an outreach plan.
  • Assist with and participate in outreach, community engagement and referrals.
  • Assist the Project Coordinator in accepting/seeking out donations for FRC use and events.
  • Under the direction of the Project Coordinator, assist volunteers at the FRC.

SCHEDULE:  Monday – Friday, 8:30 am – 5:00 pm

                 

      

SALARY:       $19.10 - $21.48 per hour, to start

 

Plus $100 monthly stipend for bilingual candidates in Spanish, Russian, Hmong, Cantonese, Farsi (Dari), Vietnamese, or Arabic.  

QUALIFICATIONS


Education and Experience:

A high school diploma or general education degree (GED) is required.

 

AA degree in Early Childhood Education, Human Development or similar field from an accredited college or university is preferred.

 

1 year (1700-hour term) experience as an AmeriCorps member in a FRC/Birth & Beyond Program is preferred.

 

Driving is an essential function of this job. As such, employees in this position will be required to furnish their own vehicle to be used in the course of employment. A valid driver’s license and minimum insurance coverage is also required. Must be at least 21 years old.

 

 

Knowledge, Skills and Abilities:

  • Must be able to read, write, speak and understand the English language.
  • Ability to establish and maintain appropriate boundaries with families.
  • Knowledge of child abuse issues.
  • Knowledge of child development and behavior
  • Ability to deliver the cuuriculum in a culturaly relative manner. 
  • Knowledge of family dynamics.
  • Knowledge of substance abuse issues.
  • Knowledge of local cultures.
  • Skill to communicate effectively in written and oral form.
  • Good organizational skills.
  • Skill to maintain a professional, confidential work environment.
  • Ability to manage multiple tasks at one time in an efficient manner.
  • Excellent problem solving skills.
  • Knowledge of Microsoft Word/Outlook
  • Ability to be culturally sensitive.
