Job Summary
General Summary of Duties : The Home Visitor position is a part of Sacred Circle Healthcare’s Family Spirit Home Visiting program. The home visitor position is seeking a dedicated and passionate worker who will be responsible for providing comprehensive, individualized home visits to families, utilizing the Family Spirit curriculum to teach lessons to parents and children from prenatal to 3 years old. This role will focus on enhancing child development, supporting positive parenting, strengthening family capacity and connecting families to community resources as needed. This position requires a compassionate individual who is committed to fostering growth and development within families. The ideal candidate is comfortable working independently, has strong organizational and data management skills, is self-motivated and adaptable, has a warm and engaging personality and enjoys providing caring support to parents and children.

To Be Determined with the availability of 7 : 45 am – 7 : 00 pm Monday through Friday. Typically, four 10- hour shifts per week. Evening and weekend work may be required depending on position requirements.

Responsibilities :
The successful candidate :
- Completes in-person training for the Family Spirit program, which requires travel out of state in March and June 2025 (all expenses paid by the clinic).
- Uses the Family Spirit lesson plans, the home visitor plans, develops, and implements individualized home visits for families on their caseload, either at the family’s home, the clinic, or another approved location.
- Follows up with referrals of potential clients to inform them about the Family Spirit program and schedule intake appointments.
- Monitors caseload to ensure all required documentation and requirements are completed on time. Ensures all enrollment and intake paperwork, consents, screenings, assessments, visits, etc. are documented appropriately within the required timeframes. Ability to use online documentation system in English.
- Assists with recruitment events in the community to educate about the Family Spirit program and its availability.
- Meets all grant requirements including documentation and data gathering for visits.
- Assists with planning and conducting community activities and events.
- Completes ASQ training and conduct screenings and assessments by the due dates and at the necessary intervals.
- Assists families with referrals to community agencies to fulfill their needs.
- Travels throughout Salt Lake County using your personal vehicle or the company vehicle (mileage reimbursement provided). Some travel to Wendover and Ibapah may also be required.
- Reads, understands, and adheres to all Sacred Circle Health Care Policies and Procedures.
- Practices and adheres to the Code of Conduct and Mission and Values Statements.
- Other Duties as Assigned.
- Fulfills patient care responsibilities as assigned that may include checking schedules & organizing patient flow.
- Prepares patients for examination and treatment.
- Takes patient histories and vital signs.
- Performs blood draws and specimen handling.
- Prepares exam and treatment rooms with necessary supplies, materials, and instruments.
- Gives injections, and applies casts, splints, and dressings, as applicable.
- Prepares and maintains supplies and equipment for treatments, including sterilization.
- Assists physicians in preparing for minor surgeries and physicals.
- Assists with scheduling of tests and treatments.
- Screens telephone calls for referral to physician.
- Provides general information on departmental policies and procedures.
- Notifies the Clinical Supervisor or Office Manager when clinical and office supplies need replenishing.
- Assists in maintaining patient files, records, and other information to contribute to accurate data gathering.
- Compiles and condenses technical and statistical data for reports and records.
- Attends required meetings and participates in committees as requested.
- Participates in professional development activities and maintains professional affiliations.
- Maintains patient confidentiality.
- Reads, understands, and adheres to all Sacred Circle Health Care Policies and Procedures.
- Practices and adheres to the Code of Conduct and Mission and Values Statements.

Minimum Requirements
EDUCATION :
A high school diploma or GED equivalent is required
EXPERIENCE :
CERTIFICATION / LICENSE :
Valid driver's license.
CPR Certification (provided upon hire).
Other :
Spanish and English language fluency.
Must pass a background check.
Preferred Requirements
EDUCATION :
Degree in child development and / or early child education.
EXPERIENCE :
One (1) year of experience working independently and efficiently managing multiple tasks.
One (1) year of experience working with children and families and / or one (1) year experience working in the medical field.

Experience working with the Native American community.
