Sacred Heart Care Center, Inc. is a private, nonprofit corporation associated with Austin’s three Catholic parishes which are committed to continued growth to meet the changing healthcare needs of the Austin community by providing comprehensive health care that encompasses the spiritual, physical, social, mental, and emotional needs of the residents and clients it services.
Our campus is comprised of a Skilled Nursing Facility serving 59 residents, 26 Assisted Living apartments , and two Adult Day Care programs.
RESPONSIBILITIES AND DUTIES
- Manage own time to accomplish assigned duties.
- Present appearance consistent with the facility dress code, wear name tag.
- Come to work as scheduled and consistently demonstrate dependability and punctuality.
- Assist new employees in following established policies and procedures.
- Assure resident rights are respected in all resident situations.
- Answer any call light/need promptly and obtain appropriate assistance as
needed.
- Respond to all emergency situations as needed, drills and real emergencies.
- Follow standard precautions and infection control practices.
- Follow SHCC Vulnerable Adult policy. Promote Resident Rights.
- Follow departmental Safety Rules and promote safety.
- Use and promote positive working relationships with co-workers and other departments (teamwork).
· Use and promote effective communication techniques with residents, families, visitors, co-workers and other health care professionals.
- Promote positive image of SHCC.
- Address resident/staff issues in private.
- Address resident concerns and take appropriate action to correct. Contact the correct supervisor if concerns become known.
- Attend mandatory in-services to improve knowledge and skills to maintain competency.
- Attend and participate in meetings and committees, collect, record and report data as needed.
- Participate in Quality Projects as needed and asked to do.
- Maintain Nursing license current; meet requirements as specified by the Board of Nursing.
- Monitor and evaluate cares provided by NAR and initiate corrective action as
necessary, complete nursing assistant evaluations as assigned.
- Accept and give shift report, including sufficient information for follow up action if necessary.
- Make nursing assistant assignments and explain as needed.
- Use critical thinking and problem solving techniques when managing care delivery.
- Correct nursing assistants as needed and report to the QA Coordinator if continued problems.
- Assure compliance with federal, state, and facility regulations.
- Performs or coordinates monitoring of rehab program, resident care audits and infection control programs on assigned wing.
- Supervise all resident care. Check dashboard to ensure that cares are completed and charted as such.
- Monitor staffing and make necessary adjustments to staff the wings.
- Develop comprehensive care plan as indicated. Ensure that care plan is followed.
- Participate in nursing audit activities as needed.
- Perform Charge Duties as assigned.
- Receive report and give report to aides and following shift.
- Deliver direct resident care.
- Accurately administer medications and treatments as ordered by the physician and document thoroughly and accurately without bias or opinion the effects of the meds or treatments.
- Assure accurate Narcotic Count following policy. (Make sure you do the count with outgoing and incoming shift).
- Destroy medications that are no longer needed according to policy.
- Exchange keys with the preceding and following shifts.
- Assure that residents receive the appropriate diet or fluid amount per physician's orders.
- Administer PRN medications and evaluate the effectiveness of that medication and document thoroughly.
- Check orders from previous shift and from rounds. Check for SBARS and complete as needed.
- Check alerts in PCC. Address as needed.
- Assure that residents are fed as indicated, monitor intake, report problems and take appropriate action so that all residents maintain adequate nutrition. Assist as needed.
- Assure that residents are walked as indicated and assist as needed.
- Assure that appropriate documentation is completed, Medicare, weekly charting, rehab charting, change in condition, follow-up to falls, incidents, etc.
- Follow pharmacy policies and procedures.
- Follow policies and procedures when doing resident treatments, wound management, and other programs and assure accurate and timely documentation to reflect what was done.
- Perform nursing duties as indicated; accucheks, vital signs, enteral feedings, trachs, caths, turning while in bed, instruction to nursing assistants as needed, etc.
- Transcribe physician's orders accurately and timely.
- Collect appropriate data prior to making call to physicians
- Assist with the transfer of a resident to the hospital or admit to the facility and complete all the necessary paperwork that is required.
- Complete referral forms with correct data
- Monitor any use of physical or chemical restraint and document as needed, monitor for monitor for alternative methods and document.
- Order new medications as needed.
Attend and participate in care conferences and updating of care plans as indicated.
- Maintain order at the nurses station.
- Maintain order on the med cart.
- Use transfer belt when transferring or ambulating resident.
- Use mechanical lift according to plan of care, manufacturer's directions and facility policy.
- Ensure that cart is stocked for next shift.
- Communicate changes in condition, orders to resident and families in timely fashion.
- Conduct transfer assessments, pain assessments, skin assessments, etc as indicated.
- Complete and document Vital Signs as needed.
- Ensure that documentation is complete: Medicare, Pain, Skin, Braden, General progress note, etc.
- Assist with training of new staff.
- Assist with meeting the needs of the residents, pick up extra hours as needed.
- Monitor staff breaks.
- Responsible for ensuring that needs of residents are met.
- Assist on other wings as needed if TMA working.
- Perform other duties as assigned by Supervisors.
