About The Role

Key Responsibilities
- Provide families with referrals and linkages to community services and resources.
- Assist families in following through with referrals and accessing necessary support.
- Collaborate with other professionals, service providers, and community partners.
- Conduct home-based meetings and follow-up visits as required.
- Stay knowledgeable about available community resources to meet families’ needs.
- Arrange or provide transportation for clients when necessary.
- Maintain professional, respectful interactions with families and their support systems.
- Document case information and communications accurately and in a timely manner.
- Participate in staff meetings, training, and professional development opportunities.
- Support safety plan monitoring and provide backup assistance to the prevention team.

Requirements
Qualifications:
- Experience in social services, child welfare, case management, or a related field preferred.
- Strong communication, problem-solving, and organizational skills.
- Ability to engage with families in their homes and connect them to appropriate resources.
- Familiarity with community services and support systems is a plus.
- Must have a valid driver’s license and reliable transportation

Salary : $50,000 - $52,000
