We are seeking a compassionate and experienced Hospice Chaplain / Spiritual Care Coordinator to provide individualized spiritual counseling and support for patients and families during end-of-life care.

What You’ll Do:
- Assess spiritual needs of hospice patients and their families and develop an individualized plan of care.
- Provide direct spiritual and emotional support through visits, counseling, and guidance.
- Conduct services of prayer, worship, and ritual, respecting the beliefs and preferences of patients and families.
- Collaborate with the interdisciplinary team and local clergy as appropriate.
- Assist with bereavement support, funeral planning, and crisis counseling.
- Help families navigate advance directives and legal or ethical concerns.
- Maintain clear and timely documentation according to agency guidelines.
What We’re Looking For:
- College degree required; Master of Divinity preferred.
- Ordination or commission from a recognized faith community.
- At least one (1) year of experience in a healthcare or hospice setting preferred.
- Strong counseling, communication, and crisis intervention skills.
- Ability to provide spiritual care to individuals of all faiths, philosophies, and cultural backgrounds.
- Valid driver's license, car insurance, and reliable transportation.
