Key Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient needs
• Develop and implement individualized care plans in collaboration with the healthcare team
• Provide counseling and emotional support to patients and families dealing with chronic illnesses, disabilities, or life transitions
• Assist patients in accessing community resources, financial aid, and support programs
• Coordinate with physicians, nurses, and other healthcare professionals to ensure seamless patient care
• Educate patients and families about available healthcare options, insurance benefits, and advanced care planning
• Maintain accurate and timely documentation using electronic medical record (EMR) systems such as Axxess and Home Health Notify
• Advocate for patients' rights and provide crisis intervention when necessary

Qualifications

Qualifications

• Master’s degree in social work (MSW) from an accredited institution
• Active State of Michigan licensure as a Social Worker (LMSW preferred)
• Minimum of one year of experience in a healthcare or home health setting
• Strong problem-solving, communication, and advocacy skills
• Experience working with diverse populations and knowledge of community resources
• Reliable transportation with a valid Michigan driver’s license
