Job Overview:
We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. As a Registered Nurse, you will provide primary care to patients in their homes, ensuring their comfort and well-being. Responsibilities:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patients' vital signs and report any changes or concerns to the healthcare team
- Provide wound care, including dressing changes and wound assessments
- Perform venipuncture for lab tests and collect specimens for analysis
- Educate patients and their families on disease management, treatment plans, and self-care techniques
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care
- Maintain accurate and up-to-date medical documentation using electronic health record systems (e.g., eClinicalWorks)
- Utilize case management skills to ensure continuity of care for patients
Valid Registered Nurse (RN) license in the state of Harrison Twp, Michigan
- Minimum of 1 year of experience in home health or a related field
- Proficiency in sterile processing techniques and infection control protocols
- Experience with venipuncture and specimen collection
- Strong understanding of human anatomy, physiology, and disease processes
- Familiarity with botulinum toxin treatment administration is a plus
- Excellent communication skills, both verbal and written
- Ability to work independently and as part of a multidisciplinary team
