Description
We are searching for a dedicated and experienced Occupational Therapist (OT) to join our thriving home health care team! In this fulfilling role, you will play a key role in assisting patients regain independence and improve their ability to participate in meaningful activities of daily living (ADLs) within their home environment. You will conduct assessments, develop individualized treatment plans, and utilize therapeutic interventions to help patients reach their goals.
Responsibilities:
- Conduct comprehensive occupational therapy evaluations to assess patients' physical, cognitive, and social needs within the home setting.
- Collaborate with patients and their families to establish individualized treatment plans that focus on improving their ability to perform ADLs such as bathing, dressing, grooming, toileting, eating, and meal preparation.
- Develop and implement a variety of therapeutic interventions, including activity modification, splinting, adaptive equipment training, and environmental modifications, to promote safety, independence, and participation in meaningful activities.
- Educate patients, caregivers, and family members on techniques and strategies to manage daily tasks and promote self-care.
- Document treatment progress, outcomes, and any changes in the patient's condition thoroughly and clearly.
- Maintain a safe and sanitary work environment and adhere to infection control protocols.
- Collaborate effectively with other members of the interdisciplinary healthcare team, including nurses, physical therapists, social workers, and physicians.
- Stay up-to-date on best practices in occupational therapy through continuing education.
Qualifications
Qualifications:
- Current Michigan state licensure as an Occupational Therapist (OT).
- Current CPR certification.
- Minimum of 2 years experience working in a home health care setting (preferred).
- Strong clinical reasoning, assessment, and intervention skills.
- Excellent communication, interpersonal, and collaboration skills.
- Proficient documentation skills.
- Ability to work independently and as part of a team.
- Compassionate and patient-centered approach to care.
- Valid driver's license with reliable transportation.
- Ability to perform job-related tasks which may require physical exertion, such as lifting, bending, and standing for extended periods.
