Position Overview
As a Physical Therapist (PT) at Safe Hands Home Health Care, you will play a critical role in helping patients restore mobility and achieve their rehabilitation goals in the comfort of their homes. You will develop personalized treatment plans, assess patient progress, and collaborate with other healthcare professionals to provide the highest standard of care.
Key Responsibilities
• Conduct thorough assessments of patients' physical condition, mobility, and functional abilities.
• Develop and implement personalized physical therapy treatment plans tailored to individual patient needs.
• Administer therapeutic exercises, manual therapy techniques, and other interventions to improve mobility, strength, and overall physical function.
• Educate patients and their families on exercises, mobility techniques, and preventive care to improve patient outcomes.
• Document patient progress and communicate changes in condition to the physician and interdisciplinary care team.
• Collaborate with other members of the care team, including RNs, LPNs, occupational therapists, and medical social workers.
• Ensure compliance with all regulatory and safety standards in home care.
• Maintain accurate and timely documentation in the Electronic Medical Record (EMR) system.
• Travel to patients' homes within your assigned service area, using your personal vehicle.
Qualifications
Qualifications
• Current and active Physical Therapist (PT) license in the state of Michigan.
• Minimum of 2 years of physical therapy experience, preferably in home health care or a related field.
• Strong knowledge of physical therapy techniques, manual therapy, and rehabilitation principles.
• Ability to work independently and efficiently in a home-based setting.
• Excellent communication, interpersonal, and organizational skills.
• Compassionate, patient-focused care approach.
• Must have a valid Michigan driver’s license and reliable transportation for home visits within the coverage area.
• Experience with EMR systems (such as Axxess or Home Health Notify) is preferred but not required.
