Position Overview
As a Registered Nurse (RN) at Safe Hands Home Health Care, you will provide exceptional care to patients in their homes. You will be responsible for assessing patients' conditions, creating and implementing care plans, and collaborating with a multidisciplinary team of healthcare professionals to ensure the highest quality of care.
Key Responsibilities
- Conduct patient assessments, including physical, mental, and emotional health.
- Develop and implement personalized care plans in collaboration with the care team.
- Administer medications and treatments as prescribed.
- Provide health education and support to patients and their families.
- Monitor and document patient progress and report any changes in condition to the physician.
- Coordinate care with physical, occupational, and speech therapists, dietitians, and medical social workers.
- Ensure compliance with all regulatory and safety standards.
- Utilize Axxess and Home Health Notify for accurate and timely documentation in the Electronic Medical Record (EMR) system.
Qualifications
Qualifications
- Current and active Registered Nurse (RN) license in the state of Michigan.
- Minimum of 2 year of nursing experience, preferably in home health care or a related field.
- Familiarity with EMR systems such as Axxess and Home Health Notify is preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a collaborative team.
- Strong organizational and time-management skills.
