Job Description:
We are seeking a compassionate and experienced Speech-Language Pathologist (SLP) to join our growing home health care team! As a home health SLP, you will play a vital role in helping patients regain or improve their communication, swallowing, and cognitive abilities in the comfort of their own homes.
Responsibilities:
- Conduct comprehensive speech, language, and cognitive evaluations for patients with a variety of conditions.
- Develop individualized treatment plans based on patient needs and physician orders.
- Provide therapy sessions to improve communication skills (verbal and non-verbal), swallowing function, and cognitive abilities.
- Educate patients, families, and caregivers on communication strategies and safe swallowing techniques.
- Document progress in patient charts and collaborate with the healthcare team to ensure coordinated care.
- Maintain a safe and professional work environment while traveling to patient homes.
- Participate in quality improvement initiatives and stay up-to-date on current best practices in speech-language pathology.
Qualifications
Qualifications:
- Master's Degree in Speech-Language Pathology from an accredited program.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA).
- State licensure to practice speech-language pathology.
- Minimum of one year of experience in a clinical setting, preferably in home health care or geriatrics.
- Strong clinical skills in adult speech, language, and swallowing disorders.
- Excellent communication, interpersonal, and documentation skills.
- Compassionate and patient demeanor with a focus on client-centered care.
- Ability to work independently, manage time effectively, and prioritize tasks.
- Valid driver's license with a safe driving record and reliable transportation.
