Job Overview
The Outpatient Case Manager will assist with accessing community-based resources, housing, employment, making appropriate referrals and linking the individual with ongoing mental and behavioral health, medical, legal, housing, financial, and other services to enhance the recovery process of our outpatient clients, in office and/or in the community. Assist clinical program staff with group therapy facilitation whenever necessary.
Responsibilities
- Meet and assess needs of clients upon referral and on an ongoing basis
- Assist clients with housing, medical care, food, employment, transportation, and other identified needs
- Assist clients reach their goals with legal/ criminal justice involvement through providing updates on progress via documentation, conference calls and in-person meetings, while maintaining and adhering to confidentiality relating to 42 CFR Part 2.
- Works with other community organizations to provide continuity of care through collaborative services and information sharing
- Maintains data for client records and funding sources as needed to verify applicable and successful service delivery
- Works with primary counselor to develop best plan for care of client and monitor implementation of the plan and progress of client and adjust plan as required
- Maintain and submit billing documentation timely as required by program supervisor
- Maintain knowledge of and adhere to agency policy and procedures
- Attend staff meetings, training sessions and workshops, as necessary
- Apply professionalism to all aspects of job and maintain good public relations as an agency representative
- Cultivate and maintain relationships with various community and private resources
- Maintain resource database and contact information
- Provide correct and appropriate clinical documentation as needed in the Electronic Health Record (EHR)
- Maintain 50% productivity as determined by EHR chart documentation and time card validation.
- Perform other assigned duties as necessary to accomplish agency mission, vision, goals, and objectives- under the direction of the Clinical Director
Requirements:
- High School Diploma or equivalent
- CDCA certification active with the state governing board
- Preferred - Minimum of 1 year of experience in behavioral health and case management field
- Must be organized, detail oriented, and can maintain confidentiality
- Must have competent oral, written, and interpersonal communication skills
- Good working knowledge of computers with familiarity of Microsoft Office
- First Aid and CPR Certification Required within 30 days of hire
- Final applicant is required to complete background check
- Final applicant is required to submit a new hire pre-employment drug screen
