Position Overview Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in Well Sky Kinnser EMR to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes.

• Valid Illinois RN license
• Valid CPR Certification
• Valid Drivers License
• Valid Auto Insurance
• Minimum 1 year home health experience
• Proficiency in Well Sky Kinnser EMR
• Strong clinical assessment and communication skills

Pay: $45 to 50 per hour OR $80,000.00 - $100,000.00 per year

Schedule Monday to Friday Weekends as needed

Expected Hours 40 to 50 (Less for part-time)

Experience Minimum 1-Year Home Health Experience Required

License / Certification Licensed in State of Illinois
