Safe Mission Hospice & Palliative Care, Inc. is looking for PER DIEM/PRN Hospice Certified Home Health Aide.
Job description/responsibilities:
As a primary caregiver, you are expected to provide support to patients' needs including but not limited to (feeding, cleaning, providing medical assistance and other activities of daily living). This position requires commute to various locations to provide individualized care to our patients.

Every single one in our team of clinicians, especially you, will play a vital role in ensuring that we provide quality and competent care. Hospice CHHA is expected to report to clinical supervisor and other members of IDG including but not limited to: Administrator, DPCS, Case manager, Registered nurse, LVN/LPN, and Social worker.
Requirements:
Active CHHA License
Minimum of one year experience in hospice and palliative in a hospice agency or similar field
Work as part of a cohesive team to meet each patient's needs
Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient locations
Ideal candidates must have excellent communication and interpersonal skills, familiarity with hospice regulations
Able to utilize EMR and other technology to communicate, chart, and update patient's needs
Great communication and listening skills
Seeking to work autonomously, while still working with a team
Must be able to lift up to 50 pounds if needed.
Must clear a criminal background check, physical and drug screening.
What is expected for this position:
Provide direct patient care under the supervision of RNCM
Assist with activities of daily living
Communicate DME and supply needs
Provides RNCM updates and changes
Charts visits and communicates consistently
Pay structure is Pay Per Visit: ($40-50)
Job Types: Part-time, Contract, Per diem
Pay: $40.00 - $50.00 per hour
